目的探讨尿视黄醇结合蛋白(retinolbindingprotein,RBP)在高血压病肾脏早期受损的诊断价值。方法高血压病组40例,其中Ⅰ期10例、Ⅱ期21例、Ⅲ期9例,正常对照组27例。取晨尿分别检测尿RBP、β2-微球蛋白(β2m)、N-乙酰-β-D氨基葡萄糖苷酶(NAG)、白蛋白(Alb)、转铁蛋白(TRF)和肌酐(Cr)。结果高血压病组尿RBP/Cr显著高于对照组,按高血压病分期,各期尿RBP/Cr亦显著增高。尿RBP/Cr与β2m/Cr、NAG/Cr、Alb/Cr、TRF/Cr均呈正相关。结论在高血压病早期会出现肾小管功能损害,尿RBP/Cr可作为诊断高血压病肾脏早期受损的敏感指标。
Objective To investigate the diagnostic value of retinol binding protein (RBP) in early renal damage of hypertensive patients. Methods 40 cases of hypertension group, including 10 cases of stage Ⅰ, 21 cases of stage Ⅱ, 9 cases of stage Ⅲ, 27 cases of normal control group. Urinary RBP, β2-microglobulin (β2m), N-acetyl-β-D-glucosaminidase (NAG), albumin, transferrin and creatinine were measured in morning urine. Results The urinary RBP / Cr in hypertension group was significantly higher than that in control group. According to the stage of hypertension, the RBP / Cr urinary urinary excretion increased significantly at each stage. Urine RBP / Cr was positively correlated with β2m / Cr, NAG / Cr, Alb / Cr and TRF / Cr. Conclusions Tubular dysfunction may occur in the early stage of hypertension, and urine RBP / Cr may be used as a sensitive index to diagnose early renal damage in hypertensive patients.
