目的探讨降调后基础抗苗勒管激素(AMH)水平对卵巢反应性的预测价值。方法选择2010年12月-2011年6月在新疆维吾尔自治区人民医院佳音生殖医学中心,因单纯输卵管因素不孕而行体外授精/卵胞浆内单精子注射-胚胎移植(IVF/ICSI-ET)的年轻患者(小于35岁)37例为研究对象,在控制性超促排卵(COH)前测定基础性激素水平,使用黄体期长方案(长效促性腺激素释放激素激动剂,GnRH-a)。降调后,月经来潮第2天测血.AMH水平,阴道B型超声检查窦卵泡计数(AFC)并记录获卵数、受精卵数、优质胚胎数、Gn使用总量等。结果降调后基础AMH水平与获卵数的相关系数为0.639(P<0.01),与AFC、受精卵数、优质胚胎数、COH中Gn使用总量相关系数分别为0.608,0.675,0.581,-0.751(P<0.05)。结论对于年轻不孕女性,降调后的基础AMH水平与AFC、获卵数、受精卵数、优质胚胎数及COH中Gn使用量有相关性,尤其是与获卵数有显著的相关性,获卵数、Gn用量与AMH有一定的依从关系,降调后的AMH水平可以作为预测卵巢反应性(OR)的指标。
Objective To investigate the predictive value of basal anti-Mullerian hormone (AMH) level on ovarian response after down-regulation. Methods From December 2010 to June 2011, we performed in vitro fertilization / intracytoplasmic sperm injection-embryo transfer (IVF / ICSI-ET) in Jiayin Reproductive Medicine Center, People’s Hospital of Xinjiang Uygur Autonomous Region, Of young patients (less than 35 years old), 37 subjects were enrolled. The basal hormone levels were measured before controlled hyperaemia (COH) using the long term luteinizing regimen (long acting gonadotropin releasing hormone agonist, GnRH-a). Down regulation, menstrual cramps on the second day of blood test.AMH levels, vaginal B-mode ultrasound examination of antral follicle count (AFC) and record the number of oocytes, fertilized eggs, high-quality embryos, Gn total use. Results The correlation coefficient between the baseline AMH level and the number of ova retrieved was 0.639 (P <0.01), and the correlation coefficients with the total amount of AFC, the number of fertilized eggs, the number of high quality embryos and the use of CO in COH were 0.608,0.675,0.581, 0.751 (P <0.05). Conclusions For young infertile women, the baseline level of AMH after down-regulation is related to the number of oocytes retrieved, the number of oocytes retrieved, the number of fertilized eggs, the number of high quality embryos and the amount of Gn in COH, especially the number of oocytes retrieved. Obtained oocytes, Gn dosage and AMH have a certain degree of compliance, down-regulated AMH levels can be used as an indicator of ovarian response (OR).
