目的:评估高密度多孔聚乙烯(Medpor)种植体与耳脑胶修复爆裂性眼眶骨折的效果。方法:回顾性分析开封市中心医院2015年1月至2018年1月爆裂性眼眶骨折80例(80眼)的临床资料,患者均采用Medpor种植体与耳脑胶进行修复,术后随访6个月。观察术后眼外肌功能改善、BCVA、眼球内陷程度、复视、眼球运动障碍及并发症。结果:术后6个月眼外肌麻痹功能障碍解除率为50.60%(27/54),眼外肌限制解除率为83.10%(118/142)。术后6个月视力(logMAR)为0.12±0.03,优于术前的0.48±0.11(n t=17.712,n P=0.000)。眼球内陷(-1.02±0.23)mm少于术前的(-3.04±0.66)mm(n t=25.850,n P=0.000)。复视及眼球运动障碍比例为11.25%和22.50%,少于术前的55.00%和98.75%(n P<0.05)。未发生Medpor板脱出、移位、排斥、感染、睑内翻、下睑退缩或其它并发症。n 结论:Medpor种植体与耳脑胶修复爆裂性眼眶骨折,能较好地恢复眼外肌功能,改善眼球内陷程度及复视,且安全性较高。“,”Objective:To evaluate the efficacy of repairing blowout orbital fracture with Medpor implant and otocerebral glue.Methods:The clinical data of 80 eyes of 80 cases of orbital blowout fracture from Jan. 2015 to Jan. 2018 in Kaifeng Central Hospital were analyzed retrospectively. All patients were repaired with Medpor implant and otocerebral glue. The patients were followed up for 6 months. The improvement of dysfunction of extraoular muscles, BCVA, enophthalmos, diplopia, oculomotor disturbance and complications were observed.Results:The recovery rate of extraocular muscle paralysis dysfunction was 50.00% (27/54), and the removal rate of limitation dysfunction of extraocular muscels was 83.10% (118/142) at 6 months after operation. The visual acuity (logMAR) was 0.12±0.03 at 6 months after operation, which was better than 0.48±0.11 before operation (n t=17.712, n P=0.000). Postoperative enophthalmos was (-1.02±0.23) mm, which was less than (-3.04±0.66) mm before operation (n t=25.850, n P=0.000). The rate of diplopia and oculomotor disturbance accounted for 11.25% and 22.50%, which were less than 55.00% and 98.75% before operation (n P<0.05). No Medpor plate prolapse, displacement, rejection, infection, entropion, lower eyelid retraction or other complication occurred.n Conclusion:Reparing blowout orbital fracture with Medpor implant and otocerebral glue can restore the function of extraocular muscle, improve the degree of eyeball invagination and diplopia, and has high safety.
