目的:探讨医院电子信息化管理系统在出院压疮患者家庭随访中的应用效果。方法:依托电子信息化管理系统建立出院未愈压疮患者随访库以及随访档案,由专业造口护理治疗师对2012年5月至2013年12月出院时未愈的103例压疮患者进行电话家庭随访,了解压疮家庭护理效果,给予针对性的护理指导,了解压疮护理效果与家庭护理方式之间的相关性以及患者满意度。结果:依托信息化进行电话家庭随访对出院未愈压疮患者的压疮管理起到了督促及促进作用,促进了压疮的转归,出院患者压疮的转归向着良性的方向发展(Mann-Whitney U=2 886.000,P<0.01)。每两小时翻身1次以及换药时使用促生肌药物这两种家庭护理方法促进压疮的良性转归(P<0.01)。患者及家属对压疮护理工作的满意度明显提高(t=-7.916,P<0.05)。结论:依托电子信息化的电话家庭随访不仅促进家庭压疮的良性转归,同时针对性的健康宣教有效预防了压疮的再次发生,提升了患者满意度,完善了延续性护理模式,体现了延伸护理服务的实用性和社会效益。
Objective: To explore the application effect of hospital electronic information management system in family follow-up of discharged pressure ulcer patients. Methods: Based on the Electronic Information Management System, 103 patients with pressure ulcers who were discharged from hospital from May 2012 to December 2013 were interviewed by professional ostomy nurses. Home follow-up to understand the effect of pressure ulcer home care, targeted care guidance to understand the relationship between pressure ulcer care and home care and patient satisfaction and patient satisfaction. Results: Home telephone follow-up by informatization played a supervisory role in the management of pressure ulcers in discharged patients without pressure ulcer healing, promoted the outcome of pressure ulcer and the benign development of pressure ulcer in discharged patients (Mann- Whitney U = 2 886.000, P <0.01). The two home-care methods, one stand-up every two hours and the use of myogenic medications during the dressing change, promote the benign outcome of pressure ulcers (P <0.01). Patients and their families were significantly more satisfied with pressure ulcer care (t = -7.916, P <0.05). Conclusion: The home-based telephony with electronic information not only promotes the benign outcome of family pressure ulcers, but also targeted health education effectively prevents the recurrence of pressure ulcers, improves patient satisfaction and perfects the continuous nursing model Extend the practicality and social benefits of nursing services.
