目的探讨联合新福菌素及奈达铂化疗在局部晚期鼻咽癌治疗中的应用价值,为临床更为有效的治疗局部晚期鼻咽癌提供参考。方法将80例局部晚期鼻咽癌患者随机分为对照组40例和试验组40例。2组患者均接受放疗治疗,放疗治疗方案应用调强适形放射治疗方案。对照组给予30 mg·m~(-2)奈达铂,静脉滴注,每周1次;试验组在对照组的基础上给予新福菌素注射剂0.4 mg+5%葡萄糖注射液500 m L,静脉滴注,伴行静脉输液4~5 h,每2 d用药1次。2组均治疗6周。比较2组患者的鼻咽部原发灶和颈部淋巴结转移灶的临床疗效以及药物不良反应的发生情况。结果治疗后,试验组和对照组的总有效率分别为67.50%(27例/40例)和45.00%(18例/40例),差异有统计学意义(P<0.05)。治疗后,咽部原发病灶面积均下降,试验组和对照组分别为(19.11±2.07),(23.6±5.51)mm;颈部淋巴结转移分别为(5.49±3.92),(9.24±3.83)mm,差异有统计学意义(P<0.05)。试验组和对照组的药物不良反应有骨髓抑制、胃肠道反应、腔黏膜反应、口干反应、皮肤反应。试验组和对照组的药物不良反应发生率分别为82.50%(33例/40例)和72.50%(29例/40例),差异无统计学意义(P>0.05)。结论新福菌素联合奈达铂治疗局部晚期鼻咽癌方案临床疗效好,安全性高。
Objective To investigate the value of combined neomycin and nedaplatin chemotherapy in the treatment of locally advanced nasopharyngeal carcinoma and provide a reference for the treatment of locally advanced nasopharyngeal carcinoma more effectively. Methods Eighty patients with locally advanced nasopharyngeal carcinoma were randomly divided into control group (40 cases) and experimental group (40 cases). The patients in both groups received radiotherapy, and the radiation therapy regimen used intensity-modulated conformal radiotherapy. The control group was given nedaplatin 30 mg · m -2 intravenously once a week. On the basis of the control group, the mice in the experimental group were treated with 0.4 mg of nebutatin injection and 500 m L of 5% glucose injection , Intravenous drip, with intravenous infusion 4 ~ 5 h, medication every 2 d. The two groups were treated for 6 weeks. The clinical efficacy and adverse reactions of nasopharyngeal primary tumor and cervical lymph node metastases in two groups were compared. Results After treatment, the total effective rate was 67.50% (27 cases / 40 cases) and 45.00% (18 cases / 40 cases) in experimental group and control group, respectively. The difference was statistically significant (P <0.05). After treatment, the area of the primary lesion in the pharynx decreased, with (19.11 ± 2.07) and (23.6 ± 5.51) mm in the test group and the control group respectively. The cervical lymph node metastases were (5.49 ± 3.92) and (9.24 ± 3.83) mm , The difference was statistically significant (P <0.05). Adverse drug reactions in the test group and the control group were bone marrow suppression, gastrointestinal reaction, luminal mucosal reaction, dry mouth reaction and skin reaction. The incidences of adverse drug reactions were 82.50% (33 cases / 40 cases) and 72.50% (29 cases / 40 cases) respectively, with no significant difference between the two groups (P> 0.05). Conclusion The combination of nebutalin and nedaplatin in the treatment of locally advanced nasopharyngeal carcinoma has good curative effect and high safety.
