目的探讨超声在诊断新生儿缺氧缺血性脑病(HIE)中的临床价值。方法选取我院新生儿病房住院窒息患儿(HIE组)83例和健康新生儿(正常对照组)30例,HIE组又可分为轻度HIE组49例,中度HIE组21例,重度HIE组13例。使用经颅彩色多普勒超声显像诊断仪分析各组脑组织形态、结构和超声显像特点,通过测定大脑中动脉的收缩及舒张期血流速度、阻力指数和搏动指数等对HIE组进行超声分级,并与临床分级进行对照分析。结果 83例均有不同程度HIE症状和体征,其超声显像特点和多普勒测定参数均有不同表现,并经头颅CT和其他相关检查证实。结论多普勒超声测定脑血流速度参数情况不仅可以反映脑灌注情况,而且可反映脑血管的收缩功能,对脑损伤的早期诊断和预后的评估具有一定的临床意义。
Objective To investigate the clinical value of ultrasound in the diagnosis of neonatal hypoxic-ischemic encephalopathy (HIE). Methods Eighty-three inpatients with asymptomatic neonates (HIE group) and 30 healthy newborns (normal control group) in neonatal ward of our hospital were selected. HIE group was divided into mild HIE group (n = 49), moderate HIE group (n = 21) HIE group of 13 cases. Transcranial color Doppler ultrasonography was used to analyze the morphological, structural and ultrastructural features of brain tissue in each group. The HIE group was measured by measuring the systolic and diastolic blood flow velocity, resistance index and pulsatility index Ultrasound grading, and clinical classification and control analysis. Results 83 cases had different degrees of symptoms and signs of HIE, and its ultrasound imaging characteristics and Doppler measured parameters have different performance, and by head CT and other related tests confirmed. Conclusion The determination of cerebral blood flow velocity parameters by Doppler ultrasound not only reflects cerebral perfusion, but also reflects the contractile function of cerebrovascular. It has some clinical significance for the early diagnosis and prognosis evaluation of brain injury.
