目的初步探讨首发以“污染/洗涤”为主要表现的强迫症(OCD)患者尾状核体积及磁共振波谱(MRS)改变特点。方法 11例临床首次确诊的以“污染/洗涤”为主要表现的OCD患者(OCD组)和与之匹配的11名健康志愿者(对照组),采用1.5 T MR测量两组尾状核体积,并采用单体素点回波成像序列,采集尾状核头的MRS图像,经系统自带软件处理,得到NAA/Cr、Cho/Cr的比值;比较两组尾状核体积及代谢指标的差异。结果 OCD组尾状核体体积均数略小于对照组,但左侧与右侧尾状核体积差异均无统计学意义。OCD组右侧尾状核头NAA/Cr、Cho/Cr高于对照组,差异有统计学意义;左侧尾状核头NAA/Cr较对照组略有增高,Cho/Cr较对照组略有减低,但差异均无统计学意义。结论首发以“污染/洗涤”为主要表现的OCD患者双侧尾状核体积无明显变化,右侧NAA/Cr、Cho/Cr值较对照组增高,左侧NAA/Cr、Cho/Cr值无明显变化。
Objective To investigate the changes of caudate nucleus volume and magnetic resonance spectroscopy (MRS) in patients with obsessive-compulsive disorder (OCD), whose first appearance was “contamination / washing”. Results The average volume of caudate nuclei in OCD group was slightly smaller than that in control group, but there was no significant difference in the volume of left caudate nucleus and right caudate nucleus. The ratio of NAA / Cr and Cho / Cr in the right caudate nucleus of OCD group was higher than that of the control group, and the difference was statistically significant. NAA / Cr in the left caudate nucleus was slightly higher than that in the control group, while Cho / Cr was slightly Reduce, but the difference was not statistically significant. Conclusions There was no significant change in bilateral caudate nucleus in the first episode of “pollution / wash” in OCD patients. NAA / Cr and Cho / Cr in right side were higher than those in control group. NAA / Cr, Cho / Cr No significant change in value.
