目的维护市民健康,照顾弱势族群,促进小区健康服务,建立民众自主健康管理并关怀慢性病患者的健康。方法依据台北市政府推动《市民健康生活照护服务计划》,2009年4月至2014年12月,在12个行政区实施服务计划。结果全市共设置918个居家型照护站及118个小区型照护站,提供市民健康生活照护客服中心主动电话关怀服务量达17万7 409通,整体系统照护服务使用量达190万6 339人次。满意度调查:有94.5%的服务对象对电话关怀服务感到满意,91.4%对家访服务感到满意,90.8%对整体的服务感到满意。结论远距照护服务可促使高血压病患从事自我行为管理,逐渐达到疾病控制,而且个案接受远距照护对其血压控制有一定效果,但仍应强化个案的量测行为。
Objective To safeguard public health, care for disadvantaged groups, promote community health services, establish people’s voluntary health management and care for the health of patients with chronic diseases. Methods According to the Taipei City Government’s promotion of “Citizens’ Healthy Living Care Service Plan”, from April 2009 to December 2014, the service plan was implemented in 12 administrative regions. Results A total of 918 home-based care stations and 118 community-based care stations were set up in the city. The number of active telephone care services provided by the Citizens’ Healthy Living Care Service Center reached 177,977 and the overall system care service usage was 1,969,639. Satisfaction survey: 94.5% of the respondents were satisfied with the telephone care service, 91.4% were satisfied with the home visit service, and 90.8% were satisfied with the overall service. Conclusion Telemedicine services can promote the self-behavior management of hypertensive patients and gradually achieve disease control. In addition, the telemedicine cases have some effect on their blood pressure control, but the measurement of individual cases should be strengthened.
