大剂量甲基强的松龙(MP)冲击疗法已广泛应用于治疗严重肾脏病,但它对肾功能的直接影响仍有争议。本文研究了MP冲击疗法(1g/d置5%葡萄糖500ml中60分种输完,共连续3天,然后口服强的松60mg/d)对15名原、继发肾病患者肾功能的影响。发现3例治疗前有活动性肾脏病、肾功能不全的肾病综合征患者MP冲击治疗后发生了一过性肾衰。2例在白蛋白-速尿治疗利尿后肾衰很快转复,1例在停MP后血清肌酐即迅速恢复到治疗前水平。作者观察了这15例患者治疗前后血清肌酐变化△Scr)与患者治疗前血清白蛋白水平及血清肌酐水平的联系,发现△Scr与血清白蛋白负相关(r=-0.899,
High-dose methylprednisolone (MP) shock therapy has been widely used to treat severe kidney disease, but its direct effect on renal function is still controversial. In this paper, we investigated the effect of MP shock therapy on the renal function of 15 patients with primary and secondary nephropathy (60 days after 1 g / d of 500% dextrose in 500 ml) for 3 consecutive days followed by oral prednisone 60 mg / d.
