本文报告5例坏死性肺炎患者痰内发现弹性蛋白纤维(Elastin fibers,EF)。男4例,女1例。除1例的年龄为34岁外,余为62~74岁。检查方法:取痰后应立即检查,取痰的脓性部分置玻片上加一滴40%KOH液,加盖玻片镜检即可见到EF,在放大400倍镜下可见呈特殊的裂端。若EF示为丛状即为阳性,若为很多单个纤维则属高度可疑。5例的痰皆有弹性蛋白的形态,组织化学及生化的特性。部分纯化纤维制备物经特殊染色所示与弹性蛋白相符,但未发现胶原。镜检所示痰内的纤维与散布在肺泡间隔,血管及呼吸性细支气管壁上和肺脓疡坏死中心内的弹性蛋白成分极为相似。电镜下痰纤维的形态与受损肺的弹性蛋白形态完全相同。制备的部分纯化纤维经弹性蛋白酶消化可完全破坏此纤维,而用缓冲液处理的则无此作用;故痰内的纤维完全可能
This article reports 5 patients with necrotizing pneumonia found in the sputum of elastin fibers (Elastin fibers, EF). 4 males and 1 female. In addition to a case of 34-year-old, I was 62 to 74 years of age. Inspection methods: take sputum should be immediately checked, take sputum purulent part of the slide glass plus a drop of 40% KOH solution, covered with a glass microscope can be seen EF, 400 magnification showed a special lens. If EF is indicated as plexiform is positive, for many single fiber is highly suspicious. 5 cases of phlegm are elastin morphology, histochemistry and biochemical characteristics. Partially purified fibrin preparations were shown to be elastin-specific, but no collagen was found. Microscopic examination showed that the fibers within the sputum were scattered in the alveolar septum, blood vessels and respiratory bronchial wall and lung abscess necrosis center of the elastin composition is very similar. The morphology of sputum fibers under electron microscopy is identical to that of damaged elastin in the lungs. Preparation of partially purified fibers by elastase digestion can completely destroy the fiber, while the buffer was no such effect; so the phlegm within the fiber is entirely possible
