目的探讨乳腺导管原位癌(DCIS)的影像学诊断方法及临床价值。方法 28例DCIS患者,入院后择期给予手术治疗,均采取X线钼靶、彩色多普勒超声及磁共振检测,比较三种影像学检测技术的诊断准确率。结果 28例患者中,X线钼靶提示钙化20例,结构紊乱6例,结节5例;彩色多普勒超声提示结节24例,导管扩张2例;磁共振提示非肿块强化灶21例。磁共振的诊断准确率高于其他两种检测,差异均具有统计学意义(P<0.05)。结论应用X线钼靶、彩色多普勒超声及磁共振检测DCIS各有优势,磁共振诊断准确率最高,三项联合检测可大大提高诊断准确率,值得临床广泛推广应用。
Objective To investigate the imaging diagnosis and clinical value of ductal carcinoma in situ (DCIS) in breast. Methods Twenty-eight DCIS patients undergoing elective surgery were admitted to hospital. All patients underwent X-ray mammography, color Doppler ultrasound and magnetic resonance imaging. The diagnostic accuracy of the three imaging techniques was compared. Results Of the 28 patients, X-ray mammography revealed calcification in 20 cases, structural disorder in 6 cases and nodules in 5 cases. Color Doppler ultrasound showed nodules in 24 cases and catheter expansion in 2 cases. Magnetic resonance imaging showed non-mass enhancement lesions in 21 cases . The diagnostic accuracy of MRI was higher than the other two tests, the differences were statistically significant (P <0.05). Conclusion The use of X-ray molybdenum target, color Doppler ultrasound and magnetic resonance DCIS have their own advantages, the highest diagnostic accuracy of magnetic resonance, the three joint detection can greatly improve the diagnostic accuracy, it is widely used in clinical promotion.
