目的探讨小剂量雌激素治疗绝经后妇女反复泌尿系感染的临床效果,以期提高临床治疗质量。方法选取2010年3月-2013年2月105例绝经后妇女反复泌尿系感染患者,将患者分成两组,对照组51例,采用常规的治疗方法,观察组54例,在对照组基础上加用小剂量雌激素,观察两组在临床效果、症状改善和子宫内膜厚度等方面的差异性,采用SPSS13.0软件进行统计分析。结果对照组症状改善率治疗两周时为69.8%、治疗4周为94.4%、治疗1个月为98.3%、治疗6个月为100.0%,观察组症状改善率治疗两周后为96.1%、治疗4周、1、6个月后均为100.0%,两组比较差异有统计学意义(P<0.05);对照组治愈率21.57%,总有效率为82.36%,观察组治愈率64.81%,总有效率为96.29%,两组比较差异有统计学意义(P<0.05);治疗前后两组除子宫内膜厚度外,其他指标比较差异均有统计学意义(P<0.05);两组治疗后在E2值、pH值、乳杆菌、阴道健康评分、阴道细胞成熟度、生活质量评分比较差异有统计学意义(P<0.05);观察组不良反应发生率5.56%,对照组无不良反应发生。结论阴道局部应用雌激素,能有效减少绝经后妇女的反复泌尿系感染。
Objective To investigate the clinical effect of low-dose estrogen treatment for recurrent urinary tract infections in postmenopausal women in order to improve the quality of clinical treatment. Methods From March 2010 to February 2013, 105 cases of recurrent urinary tract infection in postmenopausal women were divided into two groups, 51 cases in the control group, 54 cases in the observation group on the basis of the control group A small dose of estrogen was used to observe the difference in clinical effect, symptom improvement and endometrial thickness between the two groups. The cure rate of the observation group was 64.81% The total effective rate was 96.29%, the difference between the two groups was statistically significant (P <0.05); before and after treatment in addition to the thickness of the endometrial thickness, the other indicators were significantly different (P <0.05); two groups of treatment After E2 value, pH value, Lactobacillus, vaginal health score, vaginal cell maturation, quality of life scores were significantly different (P <0.05); the adverse reaction rate of 5.56% in the observation group, no adverse reactions in the control group . Conclusion The vaginal application of estrogen can effectively reduce the repeated urinary tract infection in postmenopausal women.
