目的 研究急性白血病 (AL)患者中WT1基因的表达情况及其与预后的关系。方法 用逆转录 聚合酶链反应 (RT PCR)的方法 ,研究WT1基因在 10 2例AL中的表达。结果 RT PCR检测WT1基因的敏感性为 10 -4 。WT1基因在AL所有亚型中都有表达 ,初治和复发的AL中阳性率为6 6 7% ,而在 5年长期生存者中阳性率只有 7 7% ( 4/ 5 2 )。随着完全缓解 (CR)时间的延长 ,WT1基因阳性率有所下降 ,但复发前又高度表达。WT1基因表达与否和AL的预后有关 ,WT1基因阳性组和阴性组的CR率分别为 45 8%和 83 3 %。WT1基因表达量与AL预后有很大关系 ,在AL中CR组和未完全缓解 (NR)组其相对表达量分别为 0 317和 0 80 9。结论 WT1基因在初治和复发的AL中阳性率明显高于长期生存病人。随着CR时间的延长 ,WT1基因阳性率有所下降 ,但复发前又高度表达。WT1基因是一个有用的预后指标与检测微小残留病的标记
Objective To study the expression of WT1 gene and its relationship with prognosis in patients with acute leukemia (AL). Methods Reverse transcription polymerase chain reaction (RT PCR) was used to study the expression of WT1 gene in 102 AL patients. Results The sensitivity of WT1 gene detected by RT PCR was 10 -4 . The WT1 gene was expressed in all subtypes of AL. The positive rate of AL in primary and relapsed patients was 66.7%, while the positive rate was only 77% (4/52) in 5-year long-term survivors. With the extension of complete remission (CR) time, the positive rate of WT1 gene decreased, but it was highly expressed before relapse. Whether or not WT1 gene expression is related to the prognosis of AL, the CR rates of WT1 gene positive and negative groups were 45 8% and 83 3%, respectively. The expression level of WT1 gene is closely related to the prognosis of AL. The relative expression levels of WT1 gene in CR and incomplete remission (NR) groups were 0317 and 0.89, respectively. Conclusion The positive rate of WT1 gene in primary and relapsed AL is significantly higher than that in long-term survival. With the prolongation of CR time, the positive rate of WT1 gene decreased, but it was highly expressed before relapse. WT1 gene is a useful prognostic marker and marker for detecting minimal residual disease
