目的:探讨中心静脉压(CVP)水平对脑外伤患者的早期疗效的影响。方法:根据格拉斯(GCS)评分将脑外伤患者分为3组:轻度组、中度组、重度组。分别将3组随机分为较低CVP值组(5~8cmH2O)、较高CVP值组(9~12cmH2O),根据瞳孔、神志、GCS评分及颅脑CT的改善,分3个时间(24h、72h和1周)观察其变化。结果:较低CVP值组的早期疗效优于较高CVP值组。结论:在患者CVP水平保持在5~8cmH2O时,既可保证患者脑灌注压,又不至于加重脑水肿,早期疗效较好。
Objective: To investigate the effect of central venous pressure (CVP) on the early effect of traumatic brain injury. Methods: According to the Grasse (GCS) score, the patients with traumatic brain injury were divided into 3 groups: mild group, moderate group and severe group. The three groups were randomly divided into lower CVP group (5 ~ 8cmH2O) and higher CVP group (9 ~ 12cmH2O). According to the improvement of pupil, consciousness, GCS score and CT, 72h and 1 week) observed changes. Results: The early CVP group had better early effect than the higher CVP group. Conclusion: CVP level in patients with 5 ~ 8cmH2O, can guarantee the patient’s cerebral perfusion pressure, without aggravating cerebral edema, the early effect is better.
