目的 :比较水激光和超声在根管消毒方面的杀菌效果,为临床应用提供理论依据。方法 :选择慢性根尖周炎患者60例,随机均分为3组,分别采用常规机械预备和冲洗(对照组)、常规机械预备后用水激光消毒处理(水激光组)、常规机械预备后超声荡洗根管(超声组)。用纸尖法提取根管根尖1/3的样本,用BHI培养液10倍系列稀释,分别取50μL接种于BHI血琼脂平板培养基中,37℃厌氧(80%N2,10%H_2和10%CO_2)培养24 h后计数菌落。应用SPSS19.0软件包进行单因素方差分析,比较3组病例根管预备前、后的细菌数量变化,评价三者的根管消毒效果。结果:3组在根管预备前的细菌数量无显著差异,根管预备后3组的细菌数量均有明显减少,具有显著差异。水激光组和超声组预备后的细菌数量减少显著大于对照组(P<0.05),水激光组和超声组之间无显著差异(P>0.05)。结论:经根管预备后根管内的细菌数量明显减少,应用水激光或超声后细菌减少更明显,应用水激光或超声可以增加根管的消毒效果。
Objective: To compare the bactericidal effect of water laser and ultrasound in root canal disinfection and to provide a theoretical basis for clinical application. Methods: Sixty patients with chronic periapical periodontitis were randomly divided into 3 groups: normal mechanical preparation and washing (control group), routine mechanical preparation and water laser disinfection (water laser group), conventional mechanical preparation and ultrasonography Wash the root canal (ultrasound group). One third of the apical root canal samples were extracted by paper tip method and diluted 10 times with BHI medium. Fifty microliters each were inoculated into BHI blood agar plate medium, 37 ℃ anaerobic (80% N2, 10% H 2 and 10% CO_2) cultured 24 h after counting colonies. One-way analysis of variance (ANOVA) was performed using SPSS 19.0 software package to compare the changes of bacterial counts before and after root canal preparation in three groups and to evaluate the effect of root canal disinfection. Results: There was no significant difference in the number of bacteria in the three groups before root canal preparation. The numbers of bacteria in the three groups after root canal preparation were significantly reduced, with significant differences. The number of bacteria in water laser group and ultrasound group was significantly lower than that in control group (P <0.05). There was no significant difference between water laser group and ultrasound group (P> 0.05). CONCLUSION: After root canal preparation, the number of bacteria in root canal obviously decreases. The bacterial reduction after water laser or ultrasound is more obvious. Water laser or ultrasound can increase the disinfection effect of root canal.
