目的了解上海市杨浦区男男性接触者(men who have sex with men,MSM)人群的行为特征及HIV和梅毒感染的状况。方法采用滚雪球法,在性病门诊、艾滋病自愿咨询检测(voluntary counseling and testing,VCT)门诊及部分MSM活动场所对MSM进行一对一问卷调查,并留取血液样本进行HIV、梅毒检测。结果接受调查的98人中,约一半(49.48%)的人群性伴不固定,88.78%的调查对象在最近6个月发生过男男性行为,其中未婚者发生男男性行为比例高于已婚者(2=5.13,P=0.023)。MSM人群不同年龄组间使用安全套情况差异无统计学意义(2=2.12,P=0.973),艾滋病防治知识知晓率为91.30%。最近6个月与男性发生肛交时每次都使用安全套的比例仅为33.75%。在调查的98名MSM中,HIV阳性检出率为3.06%,梅毒抗体阳性检出率为4.08%,HIV合并梅毒感染检出1例。结论艾滋病知识的知晓率和行为存在着分离现象。MSM普遍存在多性伴,安全套使用率低,这导致MSM人群一旦感染HIV,必将引起HIV在此类人群中的迅速传播,从而成为当前HIV流行的重要人群和重要因素,应积极采取有效的宣传教育和行为干预措施。
Objective To investigate the behavioral characteristics of men who have sex with men (MSM) and the status of HIV and syphilis infection in Shanghai Yangpu District. Methods The snowball method was used to conduct a one-on-one questionnaire survey on MSM in voluntary counseling and testing (VCT) clinics and some MSM venues, and blood samples were collected for HIV and syphilis testing. Results Among the 98 surveyed persons, about half (49.48%) of the population had irregular sexual partners and 88.78% of the respondents had had MSM in the past 6 months. Among them, the proportion of unmarried persons who had MSM was higher than that of married persons (2 = 5.13, P = 0.023). There was no significant difference in the use of condoms between different age groups in MSM population (2 = 2.12, P = 0.973). The awareness rate of AIDS prevention and control was 91.30%. The proportion of condom use every time anal intercourse with men in the last 6 months was only 33.75%. Among 98 MSMs surveyed, HIV positive rate was 3.06%, syphilis antibody positive rate was 4.08%, and HIV combined syphilis infection was detected in 1 case. Conclusion There is a separation between awareness and behavior of HIV / AIDS knowledge. MSM prevalence of multiple sexual partners, condom use rate is low, which led to MSM once infected with HIV, will inevitably lead to rapid transmission of HIV in these populations, which has become an important current HIV population and important factor, should actively take an effective Education and behavioral interventions.
