目的探讨脂联素(APN)、同型半胱氨酸(HCY)与慢性心力衰竭(CHF)的关系。方法 50例CHF患者,按照心功能NYHA分级分为3个亚组:CHFⅡ级组、Ⅲ级组、Ⅳ级组;选择同期健康体检者35例为对照组;所有观察对象均于住院次日清晨抽取空腹血测定APN、HCY。结果①CHF组患者血ANP、HCY浓度显著高于对照组,差异有统计学意义(P<0.05)。②CHF组患者随CHF分级的增加,血ANP、HCY水平逐步增高,差异有统计学意义(P<0.05)。③CHF组患者ANP与HCY呈正相关(r=0.583,P<0.05)。④CHF患者中ANP、HCY与左室射血分数值(LVEF)呈负相关(r=-0.029,P<0.05;r=-0.371,P<0.05)。结论 APN、HCY与CHF具有相关性,且为CHF预后的一个预测因子。
Objective To investigate the relationship between adiponectin (APN), homocysteine (HCY) and chronic heart failure (CHF). Methods Fifty patients with CHF were divided into three subgroups according to the NYHA classification of heart function: CHF ¢ ò, ¢ ógroup and ¢ ôgroup. 35 healthy subjects were selected as the control group during the same period. All the subjects were in the next morning of hospitalization Fasting blood samples taken APN, HCY. Results ① The levels of ANP and HCY in CHF group were significantly higher than those in control group (P <0.05). ② The level of ANP and HCY in CHF group increased gradually with the increase of CHF, the difference was statistically significant (P <0.05). (3) There was a positive correlation between ANP and HCY in CHF group (r = 0.583, P <0.05). ④ There was a negative correlation between ANP, HCY and left ventricular ejection fraction (LVEF) in CHF patients (r = -0.029, P <0.05; r = -0.371, P <0.05). Conclusions APN, HCY and CHF are correlated, and are a predictor of CHF prognosis.
