目的:研究窦口鼻道复合体(ostiomeatalcomplex,OMC)的CT影像解剖与慢性鼻窦炎的相关性,为OMC的研究提供影像解剖学的新思路。方法:对67例头颅冠状CT薄层扫描片进行观测,包括:中鼻道宽度,筛漏斗长度、宽度,钩突厚度、内倾角度,筛泡、鼻丘的内径,并对测量结果与鼻窦炎积分进行相关性分析。结果:67例(134侧)中钩突头端的附着部位有3种类型:筛顶型(23.13%)、纸样板型(56.72%)和中鼻甲型(20.15%),这3种类型的比例在鼻窦炎组与非鼻窦炎组有较大差异(P<0.01)。鼻窦炎组与非鼻窦炎组筛漏斗长度有显著差异(P<0.01)。筛漏斗长度及钩突角度与鼻窦炎积分之间有显著相关性。结论:OMC解剖因素(包括筛漏斗及钩突的发育等)与慢性鼻窦炎发病有密切关系。
Objective: To study the relationship between CT image anatomy and chronic sinusitis in ostiomeatal complex (OMC), and to provide a new idea of image anatomy for the study of OMC. Methods: Sixty-seven cases of coronal CT scan were observed, including the width of middle nasal canal, the length and width of sieve funnel, the thickness of uncinate process, the angle of incision, the diameter of sieve bubble and nasal cavity. The measurement results were compared with those of sinusitis Points for correlation analysis. Results: There were three types of attachment sites on the tip of the hook in 67 cases (134 sides): the top type (23.13%), the pattern type (56.72%) and the middle turbinate type (20.15%), The three types of ratio in sinusitis group and non-sinusitis group have a greater difference (P <0.01). Sinusitis group and non-sinusitis group sieve hopper length was significantly different (P <0.01). There was a significant correlation between sifter length and crooked angle and sinusitis score. Conclusion: OMC anatomical factors (including the development of screening funnel and uncinate process) are closely related to the pathogenesis of chronic sinusitis.
