目的观察老年高血压及合并2型糖尿病(T2DM)患者血管紧张素原(AGT)基因多态性与颈动脉超声变化的关系。方法选择203例住院的老年人分成高血压组(105例)、高血压合并T2DM组(38例)和对照组(健康老年体检者60人)3组。入院后测血脂,行颈动脉超声检查,采用聚合酶链反应(PCR)及限制性片段长度多态性分析法进行M235T多态性检测。结果 (1)AGT基因的TT基因型及高血压合并T2DM组的颈动脉超声异常率增高,以多支病变为主;(2)颈动脉超声有多支病变的,其血脂水平明显升高(P<0.05)。结论老年高血压和T2DM患者颈动脉超声异常发生率明显增加,而血脂水平的升高及AGT基因的TT基因型使动脉粥样硬化进一步加强。
Objective To investigate the relationship between the polymorphism of angiotensinogen (AGT) gene and the changes of carotid artery ultrasound in elderly patients with hypertension and type 2 diabetes mellitus (T2DM). Methods Totally 203 elderly patients were divided into three groups: hypertensive group (105 cases), hypertension combined with T2DM group (38 cases) and control group (healthy elderly group). Blood lipid was measured after admission, and carotid artery ultrasonography was performed. The M235T polymorphism was detected by polymerase chain reaction (PCR) and restriction fragment length polymorphism (PCR-RFLP). Results (1) The TT genotype of AGT gene and the abnormal rate of carotid ultrasonography in hypertensive patients with T2DM were mainly multi-vessel disease. (2) The multi-vessel disease of carotid artery was significantly increased P <0.05). Conclusions The incidence of abnormal carotid ultrasound in elderly patients with hypertension and T2DM is significantly increased. However, elevated serum lipids and TT genotype of AGT gene further enhance atherosclerosis.
