目的探讨乳腺超声自动容积成像技术(ABVS)与钼靶(MG)诊断乳腺肿物的应用价值。方法利用ABVS及MG对138例女性患者共170枚乳腺肿物进行检查,以病理结果为金标准,对比评价ABVS、MG以及两者结合其各自的诊断效能。结果 ABVS诊断乳腺肿物的灵敏度为98.0%、特异度为76.5%、准确度为89.4%;MG诊断乳腺肿物的灵敏度为72.5%、特异度为77.9%、准确度为74.7%;两者结合诊断乳腺肿物的灵敏度为98.0%、特异度为77.9%、准确度为90.0%。ABVS诊断乳腺肿物的灵敏度和准确度高于MG(χ~2=22.32、12.76,P<0.05),两者结合诊断乳腺肿物的灵敏度和准确度高于单用MG(χ~2=24.04、15.36,P<0.05)。ABVS、MG、两者结合的ROC曲线下面积分别为0.946、0.756、0.955。ABVS、MG乳腺病灶检出率分别为99%(168/170)、72%(123/170),ABVS乳腺病灶检出率高于MG(χ~2=41.33,P<0.05)。结论 ABVS相比MG在诊断乳腺肿物中具有较高的灵敏度和准确度,具有良好的临床应用价值。
Objective To investigate the value of automatic breast volume ultrasound (ABVS) and mammography (MG) in the diagnosis of breast masses. Methods A total of 170 breast masses from 138 female patients were examined by ABVS and MG. The pathological results were taken as the gold standard, and the diagnostic efficacy of ABVS, MG and their combination were evaluated. Results The diagnostic sensitivity of ABVS was 98.0%, the specificity was 76.5% and the accuracy was 89.4%. The diagnostic sensitivity of MG was 72.5%, the specificity was 77.9% and the accuracy was 74.7%. The combination of the two The sensitivity of breast tumor was 98.0%, specificity was 77.9% and accuracy was 90.0%. The sensitivity and accuracy of ABVS in the diagnosis of breast masses were higher than that of MG (χ ~ 2 = 22.32, 12.76, P <0.05) , 15.36, P <0.05). ABVS, MG, the area under the ROC curve of the combination of the two were 0.946,0.756,0.955. The detection rates of ABVS and MG were 99% (168/170) and 72% (123/170), respectively. The detection rate of ABVS breast lesions was higher than that of MG (χ ~ 2 = 41.33, P <0.05). Conclusion Compared with MG in the diagnosis of breast tumor, ABVS has high sensitivity and accuracy, and has good clinical value.
