目的:观察益肾泻浊化瘀方对慢性肾功能衰竭(CRF)大鼠肾脏的保护作用,并探讨其作用机理。方法:将大鼠随机分为中药益肾泻浊化瘀方组、西药依那普利组、模型组及假手术组,以5/6肾大部切除术建立慢性肾衰大鼠模型,检测各组治疗前后血尿素氮(BUN)、血肌酐(Cre);血一氧化氮(NO)、内皮素(ET)、降钙素基因相关肽(CGRP)水平及肾组织病理改变。结果:治疗后中药组和西药组BUN、Cre均较模型组明显下降(P<0.01),且中药组Cre下降幅度较西药组大(P<0.05)。模型组大鼠血ET、CGRP含量较假手术组显著升高(P<0.01),中药、西药均能有效降低ET水平,与模型组比较分别为P<0.01和P<0.05,中药组血CGRP含量下降(P<0.05)。各组血NO水平无显著差异。模型组肾组织病理改变最严重,中药组较西药组病变程度轻。结论:益肾泻浊化瘀方能有效保护CRF大鼠肾功能,且疗效优于西药依那普利,可能与其降低CRF大鼠血ET水平,减轻受损肾脏的病理改变等有关。
Objective: To observe the protective effect of Yishen Xiezhuo Huayu Recipe on the kidney of chronic renal failure (CRF) rats and to explore its mechanism. Methods: The rats were randomly divided into Yishen Xiezhuo Huayu Decoction group, western medicine enalapril group, model group and sham operation group. The rat model of chronic renal failure was established by 5/6 subtotal nephrectomy. The levels of blood urea nitrogen (BUN), serum creatinine (Cre), blood nitric oxide (NO), endothelin (ET), calcitonin gene related peptide (CGRP) and pathological changes of renal tissue in each group before and after treatment. Results: After treatment, BUN and Cre in Chinese medicine group and Western medicine group were significantly lower than those in model group (P <0.01), and Cre in Chinese medicine group was significantly lower than that in Western medicine group (P <0.05). Compared with sham-operated group, ET and CGRP levels in model group were significantly increased (P <0.01), while both traditional Chinese medicine and western medicine could effectively reduce ET level compared with model group (P <0.01 and P <0.05, respectively) Content decreased (P <0.05). There was no significant difference in blood level of NO in each group. The pathological changes of renal tissue in the model group were the most serious, and the degree of lesion in the TCM group was lighter than that in the western medicine group. Conclusion: Yishen Xiezhuo Huayu Recipe can effectively protect renal function of CRF rats and its efficacy is better than that of enalapril western medicine. It may be related to the decrease of blood ET level and the pathological changes of injured kidney in CRF rats.
