目的探讨吸毒人群人类免疫缺陷病毒感染者和艾滋病患者(HIV/AIDS)抗病毒治疗的免疫重建及病毒抑制。方法选择自2010年5月至2012年1月在广西壮族自治区龙潭医院抗病毒治疗的HIV/AIDS静脉吸毒人群作为观察组,同期的非吸毒人群作为对照组,每组60例,比较两组在基线、抗病毒治疗后第3、6、9、12个月末时CD4~+T淋巴细胞(简称CD4~+细胞)计数数值,比较基线、抗病毒治疗后的第6、12个月末的病毒载量。结果观察组完成42例,对照组完成49例,抗病毒治疗1年内两组病例CD4~+细胞均随着治疗时间延长逐步增长(P=0.000),治疗3个月时CD4~+细胞增长幅度达到最大。两组在抗病毒治疗3、6、9、12个月时CD4~+细胞计数比较,差异均有统计学意义(P<0.05)。治疗6个月时两组的病毒载量和基线比较,差异有统计学意义(P均<0.001),治疗12个月时两组的病毒载量和基线比较差异有统计学意义(P均<0.001);治疗6个月时观察组和对照组病毒抑制率分别为83.3%(35/42)和87.8%(43/49),差异无统计学意义(P=0.548);治疗12个月时病毒抑制率分别为95.2%和95.9%,差异无统计学意义(P=1.000)。结论吸毒人群HIV/AIDS病例抗病毒治疗后免疫功能得到重建,对病毒抑制效果好,但其CD4~+细胞增长小于非吸毒人群。
Objective To explore the immunological reconstruction and virus inhibition of anti-virus therapy in HIV-infected and HIV-infected patients. Methods From May 2010 to January 2012 in Longtan Hospital, Guangxi Zhuang Autonomous Region antiviral treatment of HIV / AIDS intravenous drug users as the observation group, non-drug users over the same period as a control group, 60 cases in each group, compared with two groups in the Baseline, count of CD4 ~ + T lymphocytes (CD4 ~ + cells) at the end of the 3rd, 6th, 9th, and 12th month after antiviral treatment, comparing the baseline, viral load at the 6th and 12th months after antiviral treatment the amount. Results In the observation group, 42 cases were completed and 49 cases in the control group. CD4 ~ + cells in both groups increased gradually with the prolongation of treatment time (P = 0.000) within 1 year after antiviral treatment. The increase of CD4 ~ + cells at 3 months to reach maximum. There was significant difference in CD4 ~ + cell counts between the two groups at 3, 6, 9 and 12 months after antiviral therapy (P <0.05). After 6 months of treatment, the viral load of the two groups was significantly different from baseline (all P <0.001). There was significant difference in viral load and baseline between the two groups after 12 months of treatment (P < 0.001). The virus inhibition rates in the observation group and the control group were 83.3% (35/42) and 87.8% (43/49) respectively at 6 months of treatment, with no significant difference (P = 0.548). After 12 months of treatment The virus inhibition rates were 95.2% and 95.9%, respectively, with no significant difference (P = 1.000). Conclusion The immunization function of HIV / AIDS patients after drug-resistant population was reconstructed with good effect on virus inhibition, but their CD4 ~ + cell growth was less than that of non-drug users.
