目的探讨幽门螺杆菌 (Hp)感染与冠心病心肌梗死的关系。方法测定 93例冠心病心肌梗死 (MI)患者 ,包括 5 1例急性心肌梗死 (AMI)和 4 2例陈旧性心肌梗死 (OMI)患者的Hp特异性抗体 (HpIgG、HpIgM)浓度 ,并同步观察血浆纤维蛋白原 (Fg)、血栓素B2 (TXB2 )、血液流变指标变化及其与Hp感染的相关性 ;同时与 31例冠脉造影正常者 (NC)进行比较。结果AMI组HpIgG阳性率及平均浓度 ,HpIgM阳性率及OD值明显高于NC组 (P <0 0 5 ) ,与OMI组比较差异不显著 (P >0 0 5 )。未校正冠心病危险因素时 ,HpIgG阳性与AMI具有相关关系 (OR为 3 0 4 7,95 %CI 1 0 5 5~ 8 796 ,x2 2 4 2 4 1,P =0 0 3) ,校正后二者仍有相关关系 (OR为 3 2 6 2 ,95 %CI 1 0 15 10 4 85 ,x2 2 3 94 ,P =0 0 4 7)。AMI组HpIgG、低切全血比粘度、红细胞聚集指数明显高于NC组 (P <0 0 5 ) ;TXB2 明显高于NC组 (P <0 0 1) ;Fg、血浆粘度明显高于 ,红细胞变形指数明显低于NC组 ((P <0 0 0 1) ;Fg、低切全血比粘度高于OMI组 (P <0 0 0 1) ;TXB2 、中切全血比粘度、血浆粘度、红细胞压迹高于 ,红细胞变形指数低于OMI组 (P <0 0 5 )。AMI组中Hp(+)组Fg、TXB2 明显高于Hp(- )组 (P <0 0 5 )及OMI组中Hp(+)组 (P <0 0 0 1) ;并且IgG与Fg、低切?
Objective To investigate the relationship between helicobacter pylori (Hp) infection and myocardial infarction in patients with coronary heart disease. Methods Hp-specific antibodies (HpIgG, HpIgM) were determined in 93 patients with coronary heart disease (MI), including 51 acute myocardial infarction (AMI) and 42 elderly patients with myocardial infarction (OMI) The changes of fibrinogen (Fg), thromboxane B2 (TXB2), hemorheological indexes and their correlation with Hp infection were compared with 31 cases with normal coronary angiography (NC). Results The positive rate of HpIgG and the average concentration of HpIgM in AMI group were significantly higher than those in NC group (P <0.05). HpIgG positivity was associated with AMI when the risk factors for coronary heart disease were not corrected (OR = 3047, 95% CI 1055-8 796, x22441 1, P = 0 0 3) The two were still related (OR 3262, 95% CI 1 0 15 10 4 85, x 2 2 3 94, P = 0 0 4 7).
