目的观察内镜黏膜下剥离术(ESD)治疗早期胃癌(EGC)对患者血清抗凋亡因子(Livin)、表皮生长因子(EGF)及白介素-8(IL-8)水平的影响。方法将169例EGC患者随机分成2组,研究组行内镜ESD,对照组行常规开腹切除术。比较2组患者术中出血量、手术时间及住院时间。采用ELISA法检测并比较2组手术前后血清Livin、EGF及IL-8水平表达变化。结果研究组出血量少于对照组,手术时间和住院时间均短于对照组,差异均有统计学意义(P<0.05)。治疗前,2组患者血清Livin、EGF及IL-8表达水平差异无统计学意义;治疗后2组上述血清指标水平均明显降低,且研究组低于对照组,差异均有统计学意义(P<0.05)。结论内镜ESD治疗EGC具有术中出血少、手术时间及住院时间短的优势,同时可降低血清Livin、EGF、IL-8表达水平,值得在临床进一步推广应用。
Objective To observe the effect of endoscopic submucosal dissection (ESD) on serum anti-apoptotic factor (Livin), epidermal growth factor (EGF) and interleukin-8 (IL-8) in patients with early gastric cancer (EGC) Methods 169 patients with EGC were randomly divided into two groups. The study group received endoscopic ESD and the control group received conventional open excision. The blood loss, operation time and hospital stay of two groups were compared. The changes of serum Livin, EGF and IL-8 levels before and after operation were detected by ELISA. Results The bleeding volume in study group was less than that in control group, and the operation time and hospitalization time were shorter than those in control group. The differences were statistically significant (P <0.05). Before treatment, there was no significant difference in serum Livin, EGF and IL-8 levels between the two groups (P> 0.05). After treatment, serum levels of both groups were significantly lower than those of the control group (P <0.05). Conclusion Endoscopic ESD treatment of EGC has the advantages of less intraoperative bleeding, shorter operation time and hospital stay, and lower serum levels of Livin, EGF and IL-8, which deserves further clinical application.
