目的探讨心电图ST改变对急性下壁心肌梗死梗死相关动脉(IRA)的预测价值。方法纳入发病12 h内入院的急性下壁心肌梗死患者268例,急诊行冠状动脉造影(CAG)检查根据结果将患者分为RCA组(IRA为右冠状动脉,216例)和LCX组(IRA为左回旋支冠状动脉,52例)。对2组心电图进行比较分析。结果心电图出现STⅢ↑/STⅡ↑>1、STV4R↑≥1mm、STⅢ↑>0.25 m V、STV3↓/STⅢ↑<1.2、STa VL↓>STI↓提示IRA为右冠状动脉;心电图出现STⅢ↑/STⅡ↑≤1、STV4R↑<1 mm、STⅢ↑≤0.25 m V、STV3↓/STⅢ↑≥1.2、STI↓≥STa VL↓提示IRA为左回旋支。结论体表心电图ST改变对判断急性下壁心肌梗死患者IRA有重要作用。
Objective To investigate the predictive value of ST changes on infarction related artery (IRA) in patients with acute inferior myocardial infarction. Methods A total of 268 patients with acute inferior myocardial infarction admitted to hospital within 12 hours after onset were enrolled in this study. Patients underwent emergency coronary artery angiography (CAG). According to the results, patients were divided into RCA group (IRA right coronary artery, 216 cases) and LCX group Left circumflex coronary artery, 52 cases). Two sets of ECG comparative analysis. Results ST Ⅲ ↑ / ST Ⅱ ↑> 1, STV4R ↑ ≥1mm, ST Ⅲ ↑> 0.25 mV, STV3 ↓ / STⅢ ↑ <1.2, STa VL ↓> STI ↓ indicated IRA was right coronary artery. ↑ ≤1, STV4R ↑ <1 mm, STⅢ ↑ ≤0.25 mV, STV3 ↓ / STⅢ ↑ ≥1.2, STI ↓ ≥STa VL ↓ It is suggested that IRA is the left circumflex artery. Conclusion ST changes on the body surface ECG plays an important role in judging IRA in patients with acute inferior myocardial infarction.
