目的:检测头颈部弥漫大B细胞淋巴瘤的免疫组织化学分型,并对Hans、Choi和Tally三种分型法进行比较,同时观察各蛋白的表达在各种分型法的意义。方法:收集我院2010年5月到2012年10月共103例头颈部弥漫大B细胞淋巴瘤,进行CD10、Bcl-6、MUM1、GCET1、FOXP1和LMO_2蛋白表达的免疫组织化学检测,并进行Hans、Choi和Tally三种分型。结果:103例病例中,CD10、Bcl-6、MUMl、GCET1、FOXPl和LMO_2的阳性例数分别为16例(15.53%)、68例(66.02%)、69例(66.99%)、21例(20.39%)、75例(72.82%)、50例(48.54%);Hans分型中,生发中心B细胞(GCB)型26例(25.0%),活化B细胞(ABC)型77例(75.0%);Choi分型中,GCB型25例(24.0%),ABC型78例(76.0%);Tally分型中,GCB型20例(19.4%),ABC型83例(80.6%)。CD10、MUM1、GCET1、FOXP1和LMO_2表达情况对于分型诊断结果均具有统计学意义(P<0.05),而Bcl-6的表达对于Hans和Choi分型结果无统计学意义。结论:头颈部弥漫大B细胞淋巴瘤以ABC型多见;Hans、Choi和Tally三种分型法之间无明显差别;CDl0、MUM1、GCET1、FOXP1和LMO_2的表达情况对于诊断结果均具有意义。
OBJECTIVE: To detect the immunohistochemical typing of diffuse large B-cell lymphoma in the head and neck, and to compare the typing methods of Hans, Choi and Tally, and to observe the significance of various protein typing in various typing methods. Methods: A total of 103 cases of diffuse large B cell lymphoma from May 2010 to October 2012 in our hospital were collected for immunohistochemical detection of protein expressions of CD10, Bcl-6, MUM1, GCET1, FOXP1 and LMO_2. Hans, Choi and Tally three types. Results: The positive numbers of CD10, Bcl-6, MUM1, GCET1, FOXP1 and LMO_2 in 103 cases were 16 cases (15.53%), 68 cases (66.02%), 69 cases (66.99%), 21 cases There were 26 cases (25.0%) of germinal center B cells (GCB) and 77 (75.0%) cases of activated B cells (ABC) in Hans classification, with 75 cases (72.82%) and 50 cases ). In Choi classification, there were 25 cases (24.0%) of GCB type and 78 cases (76.0%) of ABC type. Tally type GCB type 20 cases (19.4%) and ABC type 83 cases (80.6%). The expression of CD10, MUM1, GCET1, FOXP1 and LMO_2 were both statistically significant (P <0.05), while the expression of Bcl-6 was not statistically significant for Hans and Choi. CONCLUSION: The ABC type is common in diffuse large B-cell lymphoma of head and neck. There is no significant difference between the three typing methods of Hans, Choi and Tally. The expression of CD10, MUM1, GCET1, FOXP1 and LMO_2 has the same significance.
