目的探讨良性前列腺增生(BPH)和前列腺癌(PCa)的扩散加权成像(DWI)和表观扩散系数(ADC)图表现,及对PCa的鉴别诊断价值。方法分析18例BPH和25例PCa的DWI和ADC图表现,并分别测量癌肿区的ADC值及BPH的外周带(PZ)及中央腺体(CG)的ADC值,比较其间是否存在差异。结果BPH的DWI和ADC图上PZ信号近似均一,CG信号欠均匀。PCa在DWI上呈高信号,ADC图呈低信号,均能直观显示癌灶范围。受侵犯的精囊和骨盆骨转移灶DWI上呈高信号,ADC图呈低信号。PCa的ADC值[(0.49±0.09)×10-3mm2/s]与BPH的PZ及CG的ADC值[(1.27±0.14)×10-3mm2/s、(0.96±0.14)×10-3mm2/s]比较,差异均有统计学意义(t值分别为-52.46、-31.49,P值均<0.01),PCa的ADC值与BPH的PZ无重叠,与CG也仅有少量重叠(1/127,0.7%)。结论DWI和ADC图可显示PCa位置、范围及周围侵犯、转移情况;根据DWI和ADC图上病变的信号特点及ADC值可以鉴别BPH和PCa。
Objective To investigate the diffusion weighted imaging (DWI) and apparent diffusion coefficient (ADC) patterns of benign prostatic hyperplasia (BPH) and prostatic carcinoma (PCa) and the differential diagnosis of PCa. Methods The DWI and ADC maps of 18 cases of BPH and 25 cases of PCa were analyzed. The ADC value of cancer area and the ADC value of peripheral zone (PZ) and central gland (CG) of BPH were measured, and the differences were compared. Results The PZ signals on DWI and ADC maps of BPH were approximately uniform and the CG signals were not uniform. PCa showed a high signal on DWI, ADC showed a low signal, were able to visually show the scope of cancer. Invasive seminal vesicles and pelvic bone metastases DWI showed high signal, ADC showed low signal. ADC values of PCa [(0.49 ± 0.09) × 10-3mm2 / s] and ADC values of PZ and CG [(1.27 ± 0.14) × 10-3mm2 / s, 96 ± 0.14) × 10-3mm2 / s], the difference was statistically significant (t = -52.46, -31.49, P <0.01) PZ BPH no overlap, and only a small amount of CG overlap (1 / 127,0.7%). Conclusion The DWI and ADC maps can show the location, range and surrounding invasion and metastasis of PCa. According to the signal features and ADC value of DWI and ADC maps, BPH and PCa can be identified.
