目的探讨2型糖尿病(T2DM)合并冠心病患者血栓前状态(PTS)与ALDH2基因多态性的相关性。方法将140例T2DM合并冠心病患者作为研究组,189例健康检查者作为对照组。采用全自动生化仪测定生化指标;采用全自动血凝分析仪检测纤维蛋白原(FBG)水平、活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)、D-二聚体(DD);采用Baio基因芯片图像分析软件V 2.0对ALDH2基因进行图像扫描与数据分析,输出检测结果。采用logistic回归方法分析ALDH2基因多态性与T2DM合并冠心病PTS之间的相关性。结果与对照组比较,T2DM合并冠心病组的PT、APTT值均下降,而FBG、D-DD水平明显升高。线性回归分析显示,D-DD与甘油三酯(TG)以及血清胱抑素C水平密切相关;FBG与PLT、糖化血红蛋白(Hb Alc)、血清胱抑素C水平密切相关;差异均有统计学意义(P<0.05)。ALDH2两个等位基因中野生型(ALDH2*1,*504Glu)、突变型(ALDH2*2,*504Lys)与糖尿病合并冠心病患者PTS相关指标无相关性。结论 T2DM合并冠心病组的PT、APTT值均下降,而FBG、D-二DD水平明显升高,提示T2DM合并冠心病患者处于PTS;显示PLT、Hb Alc、血清胱抑素C、TG等因素是糖尿病合并冠心病患者处于PTS的主要危险因素;发现T2DM合并冠心病患者PTS与ALDH2基因多态性无显著相关性。
Objective To investigate the relationship between prothrombotic state (PTS) and ALDH2 gene polymorphism in patients with type 2 diabetes mellitus (T2DM) and coronary heart disease (CHD). Methods 140 cases of T2DM with coronary heart disease patients as the study group, 189 cases of health check-up as a control group. The biochemical parameters were measured by automatic biochemical analyzer. The levels of fibrinogen (FBG), activated partial thromboplastin time (APTT), prothrombin time (PT) and D-dimer ); Baio gene chip image analysis software V 2.0 ALDH2 gene image scanning and data analysis, the output test results. Logistic regression analysis of ALDH2 gene polymorphism and T2DM with coronary heart disease PTS correlation. Results Compared with the control group, the levels of PT and APTT in T2DM patients with coronary heart disease decreased, while the levels of FBG and D-DD increased significantly. Linear regression analysis showed that D-DD was closely related to triglyceride (TG) and serum cystatin C. FBG was closely related to PLT, Hb Alc, and serum cystatin C. The differences were statistically significant Significance (P <0.05). There was no correlation between the ALDH2 * 1, * 504Glu and ALDH2 * 2 * 504Lys in ALDH2 allele and the PTS related index in patients with diabetes mellitus complicated with coronary heart disease. Conclusions The levels of PT and APTT in T2DM patients with coronary heart disease were decreased, while the levels of FBG and D-DD in patients with T2DM complicated with coronary heart disease were significantly higher than those in patients with T2DM and coronary heart disease, suggesting that PLT, Hb Alc, serum cystatin C, TG and other factors Is a major risk factor for PTS in diabetic patients with coronary heart disease. There was no significant correlation between PTS and ALDH2 gene polymorphism in patients with T2DM and coronary heart disease.
