目的 探讨胃MALT淋巴瘤和弥漫性大B细胞淋巴瘤演进中t(11;18)(q21;q21)与幽门螺杆菌(HP)感染的关系, 以及检测API2-MALT1融合基因的意义。方法 47例胃淋巴瘤病例(MALT淋巴瘤31例,弥漫性大B细胞淋巴瘤16例),经 复查诊断后,用RT PCR和巢式PCR,检测肿瘤组织中API2-MALT1融合基因的表达,用半巢式PCR和特殊染色检查HP感染 情况。根据淋巴瘤类型及融合基因检测结果将病例分组,观察各组病例HP感染的差异。结果 47例胃淋巴瘤中20例API2 - MALT1融合基因mRNA检测阳性,包括16例MALT淋巴瘤和4例弥漫大B细胞淋巴瘤。HP感染检出率:API2- MALT1阳性 组为25%(5/20),API2- MALT1阴性组25.92%(7/27)。统计学分析表明两组间差异无显著性。2例API2 MALT1融合基因 检测阴性的胃MALT淋巴瘤病例经抗HP治疗后病情稳定。结论 胃MALT淋巴瘤和DLBCL中API2- MALT1mRNA表达与 胃淋巴瘤HP感染无明显相关,API2- MALT1融合基因可能成为胃MALT淋巴瘤抗HP治疗效果不良的预测因子。
Objective To investigate the relationship between t (11; 18) (q21; q21) and Helicobacter pylori (HP) infection in the evolution of gastric MALT lymphoma and diffuse large B cell lymphoma and the significance of detecting API2-MALT1 fusion gene. Methods 47 cases of gastric lymphoma (31 cases of MALT lymphoma and 16 cases of diffuse large B cell lymphoma) were examined. The expression of API2-MALT1 fusion gene in tumor tissue was detected by RT PCR and nested PCR. Check HP infection with semi-nested PCR and special staining. According to lymphoma type and fusion gene test results, the patients were divided into groups to observe the difference of HP infection in each group. Results The mRNA of 20 API2 - MALT1 fusion genes in 47 cases of gastric lymphoma was positive, including 16 cases of MALT lymphoma and 4 cases of diffuse large B - cell lymphoma. The detection rate of HP infection was 25% (5/20) in API2-MALT1 positive group and 25.92% (7/27) in API2-MALT1 negative group. Statistical analysis showed no significant difference between the two groups. Two cases of gastric MALT lymphoma with negative API2 MALT1 fusion gene test were stable after anti-HP treatment. Conclusion The expression of API2-MALT1mRNA in gastric MALT lymphoma and DLBCL has no significant correlation with HP infection of gastric lymphoma. The API2-MALT1 fusion gene may be a predictor of the poor response to gastric MALT lymphoma.
