目的系统评价体位变换对结肠息肉及腺瘤检出率的影响,为临床结肠镜检查提供指导。方法检索Embase、Pub Med、The Cochrane library和CBM数据库(检索时间均从建库至2016年10月),根据纳入和排除标准筛选文献,按Cochrane系统评价方法评价纳入研究的质量,并进行描述性分析。结果共纳入6篇文献,包括3篇随机平行对照试验及3篇随机交叉试验。结果显示,4篇研究表明结肠镜检查退镜过程中体位变换可提高息肉检出率,其中3篇研究提示体位变换可进一步提高腺瘤检出率。2篇随机平行对照试验表明体位变换并不能显著提高腺瘤检出率。结论现有证据表明,在结肠镜检查退镜过程中采取体位变换对息肉及腺瘤检出率的影响尚无定论。因此,在今后更多的高质量研究证明之前,体位变换尚不能常规应用于结肠镜检查。
Objective To systematically evaluate the effect of body position change on the detection rate of colon polyps and adenomas and provide guidance for clinical colonoscopy. Methods The databases of Embase, Pub Med, The Cochrane library and CBM were retrieved from the database to the end of October 2016. The literature was screened based on the inclusion and exclusion criteria. The quality of the included studies was assessed according to the Cochrane systematic review and descriptive analysis. Results A total of 6 articles were included, including 3 randomized controlled trials and 3 randomized crossover trials. The results showed that 4 studies showed that the position change during colonoscopic retrosigmation can improve the detection rate of polyp. Three studies suggest that the position change can further improve the detection rate of adenoma. Two randomized parallel controlled trials showed that postural changes did not significantly increase adenoma detection rates. Conclusions The available evidence suggests that the effect of postural change on the detection rate of polyps and adenomas during colonoscopic debridement is inconclusive. As a result, body mapping may not be routinely used for colonoscopy until more high-quality studies prove in the future.
