目的:探讨鼻、鼻窦内翻性乳头状瘤中人乳头状瘤病毒(HPV)和EB病毒的感染率及临床意义。方法:用聚合酶链反应(PCR)方法检测30例有随访记录的鼻、鼻窦内翻性乳头状瘤及恶变的石蜡包埋组织标本中的人乳头状瘤病毒和EB病毒基因。结果:30例中检出HPV20例(66.7%),EB病毒12例(40%),其中9例乳头状瘤恶变组织中检出HPV单独感染4例(全部为HPV_(16)型),EB病毒单独感染4例,两者混合感染1例,即9例乳头状瘤恶变组织中100%存在肿瘤病毒感染;10例鼻息肉中则未检出HPV和EB病毒。EB病毒感染与鼻、鼻窦内翻性乳头状瘤的复发具有相关性。结论:鼻、鼻窦内翻性乳头状瘤不仅与HPV感染有关,可能与EB病毒也有关;HPV_(16)与内翻性乳头状瘤癌变有关,EBV与复发有一定相关。
Objective: To investigate the infection rate and clinical significance of human papillomavirus (HPV) and Epstein-Barr virus in nasal and sinus inverted papilloma. Methods: Polymerase chain reaction (PCR) was used to detect human papillomavirus (HPV) and Epstein-Barr virus (EBV) genes in 30 cases of nasal and sinus papilloma and malignant paraffin-embedded tissue samples. Results: 20 cases (66.7%) were detected in 30 cases and 12 cases (40%) were Epstein-Barr virus (EBV). Nine cases of HPV infection were detected in 9 cases of papilloma (all HPV type (16) Infected with virus alone was found in 4 cases. One case was mixed infection, that is, 100% of the 9 cases had tumor virus infection in malignant tissue of papilloma. HPV and Epstein-Barr virus were not detected in 10 cases of nasal polyps. Epstein-Barr virus infection is associated with the recurrence of nasal and sinus inverted papilloma. CONCLUSION: Inverted papilloma of nose and sinus are not only related to HPV infection but also related to Epstein-Barr virus. HPV16 is associated with canceration of inverted papilloma, and EBV is related to relapse.
