目的通过检测环氧合酶2(COX2)在不同胰腺组织中的表达,探讨COX2在胰腺癌发生发展过程中的作用及与其生物学行为和预后的关系。方法应用免疫组织化学方法检测47例胰腺癌,12例胰腺导管上皮内肿瘤(PIN)和10例正常胰腺组织中COX2的表达。结果在10例正常胰腺组织中COX2阳性表达2例;COX2在PIN和胰腺癌中均呈高表达,分别为75%和68%,明显高于正常胰腺组织(P<0.05)。COX2在高分化、中分化和低分化胰腺癌组织中阳性表达率依次增高,分别为40%、69%和81%,但仅高分化与低分化组间差异有显著性(P<0.05);有淋巴结转移组COX2的阳性率为84%,明显高于无淋巴结转移组的50%(P<0.05)。COX2的表达与胰腺癌的部位、大小、是否浸润周围组织和是否发生远处转移无关(P>0.05),但与病人术后的生存期有关。生存超过1年组COX阳性表达率为38%,明显低于生存期不足1年组的79%,差异有显著性(P<0.05)。结论COX2在PIN和胰腺癌组织中高表达与胰腺癌的发生密切相关,对预测胰腺癌的分化程度、是否易发生淋巴结转移及病人的预后均有一定的作用,在临床上是具有较好指导意义的分子生物学指标。
Objective To detect the expression of cyclooxygenase 2 (COX2) in different pancreatic tissues and to explore the role of COX2 in the pathogenesis of pancreatic cancer and its relationship with its biological behavior and prognosis. Methods Immunohistochemistry was used to detect the expression of COX2 in 47 cases of pancreatic cancer, 12 cases of pancreatic ductal intraepithelial neoplasia (PIN) and 10 cases of normal pancreatic tissue. Results There were 2 cases of COX2 positive expression in 10 cases of normal pancreatic tissue. COX2 was highly expressed in PIN and pancreatic carcinoma, which were 75% and 68% respectively, which were significantly higher than that in normal pancreatic tissue (P <0.05). The positive expression rates of COX2 in well differentiated, moderately differentiated and poorly differentiated pancreatic cancer tissues were 40%, 69% and 81% respectively, but there was a significant difference between well differentiated and poorly differentiated pancreatic cancers (P <0.05). The positive rate of COX2 in patients with lymph node metastasis was 84%, which was significantly higher than that in patients without lymph node metastasis (P <0.05). The expression of COX2 was not associated with the location and size of pancreatic cancer, whether infiltrating the surrounding tissue and whether there was distant metastasis (P> 0.05), but related to the survival time of patients. The positive expression rate of COX was 38% in survival more than 1 year, which was significantly lower than 79% in survival less than 1 year group, the difference was significant (P <0.05). Conclusions The high expression of COX2 in the tissues of PIN and pancreatic cancer is closely related to the occurrence of pancreatic cancer and plays a role in predicting the degree of differentiation of pancreatic cancer, whether it is prone to lymph node metastasis and the prognosis of patients, which is of clinical significance Molecular biology indicators.
