目的评价三种不同频次的握力锻炼模式对终末期肾病(ESRD)患者内瘘功能的影响。方法采用随机对照临床试验的研究设计,对准备行动静脉内瘘形成术的ESRD患者进行内瘘功能锻炼的知识培训。培训时即将患者随机分为三组进行握力锻炼干预。通过三种频次的握力锻炼干预(A组12例,握小球每天3次;B组12例,握小球每天5次;C组10例,握小球每天7次)。遴选出既可以有效增加ESRD患者内瘘直径、静脉血流量及功能锻炼依从性,又降低其循环系统并发症发生率的内瘘功能锻炼方式。结果握力锻炼干预后,三组握力均较干预前增加(P<0.05),半年后,三组研究对象超声下内瘘吻合口最大静脉直径及血流量较做瘘1个月时有向上趋势,但无统计学差异(P>0.05)。结论每天3次的握力锻炼即可增加ESRD患者坚持内瘘功能锻炼的依从性及安全性。
Objective To evaluate the effect of three different frequency grip exercises on the fistula function in patients with end-stage renal disease (ESRD). Methods A randomized controlled clinical trial design was used to train knowledge on fistula functional exercise in patients with ESRD who were scheduled for arteriovenous fistula formation. The patients were randomly divided into three groups for Grip Training exercise intervention. Intervention was performed by three frequency grip exercises (group A, 12 cases, small ball 3 times a day; group B, 12 cases, small ball 5 times a day; group C, 10 cases, small ball 7 times a day). We selected the fistula functional exercise that can effectively increase the fistula diameter, venous flow and functional exercise compliance, and reduce the incidence of circulatory complications in patients with ESRD. Results Grip strength of three groups increased after intervention (P <0.05). After six months, the maximal vein diameter and blood flow of anastomotic fistula in three groups were upward trend compared with that of fistula at one month, But no statistical difference (P> 0.05). Conclusion Three times a day of grip exercises can increase compliance and safety of patients with ESRD insist on fistula functional exercise.
