目的分析白蛋白(ALB)、总胆红素(TBIL)、胆汁酸(TBA)联合检测在肝硬化与病毒性肝炎临床诊断中的应用价值。方法选取2013年8月至2014年8月于龙岗中心医院接受治疗的80例肝硬化、病毒性肝炎患者作为研究对象,其中肝硬化38例为A组,病毒性肝炎42例为B组。同时选取40例正常健康人作为对照组,采用循环酶速率法与比色法检测三组对象血清ALB、TBA及TBIL水平,分析三者联合检测在肝硬化与病毒性肝炎中的应用价值。结果 A组血清ALB水平为(26.2±5.6)g/L,明显低于B组与对照组(P<0.05)。A组血清TBA水平为(61.3±58.3)μmol/L,TBIL水平为(75.1±91.3)μmol/L,与B组、对照组比较差异有统计学意义(P<0.05)。ALB检测,A组阳性率为94.7%,高于B组的26.2%(P<0.05)。TBA、TBIL单向检测A组阳性率均为89.5%,同样高于B组,且三项联合检测A组阳性率为68.4%,显著高于B组的2.4%,组间比较差异有统计学意义(P<0.05)。结论在肝硬化与病毒性肝炎患者的鉴别诊断中,采取ALB、TBIL、TBA三项联合检测,参考价值高,准确度高,值得推广。
Objective To analyze the clinical value of combined detection of albumin (ALB), total bilirubin (TBIL) and bile acid (TBA) in the clinical diagnosis of cirrhosis and viral hepatitis. Methods Eighty patients with cirrhosis and viral hepatitis who were treated in Longgang Central Hospital from August 2013 to August 2014 were enrolled in this study. Among them, 38 patients with liver cirrhosis were Group A, and 42 patients with viral hepatitis were Group B patients. At the same time, 40 healthy controls were selected as the control group. Serum ALB, TBA and TBIL levels were detected by cyclotase assay and colorimetric assay in three groups of patients. The value of combined detection of these three markers in cirrhosis and viral hepatitis was analyzed. Results Serum ALB level in group A was (26.2 ± 5.6) g / L, which was significantly lower than that in group B and control group (P <0.05). The level of serum TBA in group A was (61.3 ± 58.3) μmol / L and the level of TBIL was (75.1 ± 91.3) μmol / L, which was significantly different from that in group B and control group (P <0.05). The ALB positive rate in group A was 94.7%, which was higher than that in group B (26.2%, P <0.05). The positive rates of TBA and TBIL in group A were 89.5%, which were also higher than those in group B, and the positive rates of group A and group A were 68.4% and 68.4% respectively, which were significantly higher than those in group B Significance (P <0.05). Conclusions In the differential diagnosis of patients with cirrhosis and viral hepatitis, the combination of ALB, TBIL and TBA is of high reference value and high accuracy, which is worthy of promotion.
