目的观察成人隐匿性自身免疫性糖尿病(LADA)患者胰岛β细胞功能及血清白介素6(IL-6)和肿瘤坏死因子α(TNF-α)水平的变化,探讨细胞因子在LADA发病中的作用。方法分别测定LADA患者32例、2型糖尿病(T2DM)患者30例、健康人(NC)30名的血清胰岛素(Ins)、C-肽(C-P)和IL-6、TNF-α的水平。结果 LADA组空腹、餐后Ins和C-P均明显低于T2DM组(P<0.01或P<0.05);NC组、T2DM组、LADA组的IL-6和TNF-α水平依次增高(P<0.05或P<0.01)。结论 LADA患者的胰岛β细胞功能损伤比T2DM患者严重。IL-6、TNF-α可能参与了LADA的自身免疫过程及T2DM的免疫炎症过程。
Objective To observe the changes of pancreatic β-cell function and the levels of serum interleukin 6 (IL-6) and tumor necrosis factor α (TNF-α) in adults with latent autoimmune diabetes (LADA) and explore the role of cytokines in the pathogenesis of LADA. Methods Serum levels of Ins, C-P, IL-6 and TNF-α in 32 patients with LADA, 30 with type 2 diabetes mellitus (T2DM) and 30 healthy individuals (NC) Results The fasting, postprandial ins and CP of LADA group were significantly lower than those of T2DM group (P <0.01 or P <0.05). The levels of IL-6 and TNF-α in NC group, T2DM group and LADA group were increased P <0.01). Conclusion The function of islet β cells in patients with LADA is more serious than those in patients with T2DM. IL-6, TNF-α may be involved in the autoimmune process of LADA and the immune inflammatory process of T2DM.
