目的分析百色地区结核分枝杆菌(MTB)利福平耐药相关基因rpoB的突变特征。方法收集128例结核病患者痰液标本,采用改良罗氏培养基分离培养结核分枝杆菌,绝对浓度法检测利福平耐药性,PCR技术检测耐药结核分枝杆菌rpoB基因序列并测序分析利福平耐药相关基因rpoB的突变特征。结果分离培养获得98例MTB,98例MTB出现36株利福平耐药,耐药率为36.73%,36株耐利福平菌株中有27株rpoB基因发生突变,基因突变率为75.00%,其中516位点突变率为3.70%(1/27),531位点突变率为59.26%(16/27),526位点突变率为29.63%(8/27),533位点突变率为7.41%(2/27)。突变热点依次为rpoB531、rpoB526和rpoB533。结论百色地区耐利福平结核分枝杆菌耐药性产生的主要分子机制是因为rpoB的基因发生突变。
Objective To analyze the mutation characteristics of rpoB gene in rifampicin-resistant Mycobacterium tuberculosis (MTB) in Baise area. Methods The sputum specimens of 128 patients with tuberculosis were collected. Mycobacterium tuberculosis was isolated and cultured with modified Roche medium. The drug resistance of rifampicin was detected by absolute concentration method. The sequence of rpoB gene was detected by PCR and sequenced. Mutational characteristics of rpoB gene. RESULTS: 98 MTBs were isolated and cultured, and 36 strains of rifampicin resistant to MTB were found in 98 cases. The resistance rate of 36 strains was 36.73%. There were 27 rpoB gene mutations in 36 strains of rifampin-resistant strains. The gene mutation rate was 75.00% Among them, the mutation rate of 516 site was 3.70% (1/27), the mutation rate of 531 site was 59.26% (16/27), the mutation rate of 526 site was 29.63% (8/27), the mutation rate of 533 site was 7.41 % (2/27). Mutant hot spots were rpoB531, rpoB526 and rpoB533. Conclusions The main molecular mechanism of resistance to rifampicin-resistant Mycobacterium tuberculosis in Baise area is that the gene of rpoB is mutated.
