目的:研究乳腺浸润性导管癌中CD74的表达及其临床意义。方法:用免疫组化技术检测45例乳腺浸润性导管癌组织中CD74,雌激素受体(ER),孕激素受体(PR)及c-erbB-2的表达,分析CD74表达与ER,PR及c-erbB-2表达及临床病理特征和预后的关系;观察乳腺癌MB-MDA-435细胞(高侵袭性),MCF-7细胞(低侵袭性)和转染CD74表达质粒的MCF-7细胞中CD74蛋白的表达,并用Transwell小室检测3种细胞的侵袭能力。结果:乳腺浸润性导管癌中,CD74的表达量与病理分级有关(P<0.01),癌细胞分化愈差则CD74表达愈高;CD74在受体阳性组中的表达率明显低于受体阴性组(P<0.01);三阴乳腺癌中CD74的阳性表达率明显高于非三阴乳腺癌组(P<0.01);CD74阳性患者淋巴结转移率明显高于阴性表达者(P<0.01);CD74表达阳性与否与患者的5年生存率未见明显关系性。高侵袭性MB-MDA-435细胞CD74蛋白表达量明显高于低侵袭性MCF-7细胞;MCF-7细胞转染CD74表达质粒后侵袭能力明显增强。结论:CD74可能在乳腺癌的侵袭和淋巴结转移中起着重要作用,并有可能成为一种潜在的高侵袭性乳腺癌特别是三阴乳腺癌的标志物。
Objective: To study the expression of CD74 in invasive ductal carcinoma of the breast and its clinical significance. Methods: The expressions of CD74, estrogen receptor (ER), progesterone receptor (PR) and c-erbB-2 in 45 cases of invasive ductal carcinoma of the breast were detected by immunohistochemistry. The expressions of CD74, The relationship between the expression of c-erbB-2 and the clinicopathological characteristics and prognosis of MCF-7 cells was observed. The expression of c-erbB-2 in MCF-7 cells The expression of CD74 protein in the cells was measured by Transwell chamber. Results: The expression of CD74 in breast invasive ductal carcinoma was related to the pathological grade (P <0.01). The higher the differentiation of cancer cells, the higher the expression of CD74. The expression of CD74 in the positive group was significantly lower than that in the negative (P <0.01). The positive rate of CD74 in triple negative breast cancer was significantly higher than that in non - triple negative breast cancer (P <0.01). The lymph node metastasis rate in CD74 positive patients was significantly higher than that in negative patients (P <0.01). No positive relationship was found between CD74 positive expression and patient 5-year survival rate. The expression of CD74 protein in highly aggressive MB-MDA-435 cells was significantly higher than that in low invasive MCF-7 cells. The invasion ability of MCF-7 cells transfected with CD74 expression plasmid was significantly enhanced. Conclusion: CD74 may play an important role in the invasion and lymph node metastasis of breast cancer and may become a potential marker of highly aggressive breast cancer, especially triple-negative breast cancer.
