目的调查福建省尤溪、永泰与平和等3县并殖吸虫病疫源地情况。方法在福建省尤溪、永泰与平和等3县各选取1个村作为调查点。采用ELISA法检测部分人群的并殖吸虫IgG抗体,并调查部分居民食蟹史。现场采集标本,剖检淡水螺类,检查并殖吸虫尾蚴感染情况;以双筛水洗法或直接压片法调查并殖吸虫囊蚴感染情况;采集猫、狗粪便,以水洗过筛沉淀法检查并殖吸虫虫卵。用分离自以上3地蟹类的并殖吸虫囊蚴进行人工动物感染实验。同时观察疫源地的自然生态环境。结果福建省尤溪县、永泰县和平和县等3地发现斯氏并殖吸虫、卫氏并殖吸虫和三平并殖吸虫(原名三平正并殖吸虫)。尤溪县畔岭村、永泰县赤水村居民血清抗体阳性率分别为6.8%(21/309)和6.8%(9/133)。查见淡水螺类4种,其中拟钉螺属2种和拟小豆螺属1种为斯氏并殖吸虫新的第一中间宿主。查见淡水蟹类4种,其中南海溪属1种为斯氏并殖吸虫新的第二中间宿主。尤溪县的拟钉螺和溪蟹的斯氏并殖吸虫感染率分别为2.1%(27/1 344)和92.1%(58/63),蟹感染指数为171.9,终末宿主动物粪便并殖吸虫卵阳性检出比例为1/7。永泰县的拟小豆螺和拟钉螺的斯氏并殖吸虫感染率分别为0.6%(4/690)和0.1%(2/2 330),溪蟹感染率为18.2%(18/99),蟹感染指数9.12。平和县的放逸短沟蜷和溪蟹的并殖吸虫感染率分别为0.3%(3/1 092)和44.9%(35/78),蟹感染指数为6.16。结论福建省尤溪、永泰与平和等3地发现卫氏并殖吸虫、三平并殖吸虫和斯氏并殖吸虫,是高度感染或超高度感染的并殖吸虫病疫源地。
Objective To investigate the origin of paragonimiasis in three counties of Fujian province, Youxi, Yongtai and Pinghe. Methods One village was chosen as the survey site in 3 counties of Youxi, Yongtai and Pinghe in Fujian Province. IgG antibody against Paragonimus gonorrhoeae in some populations was detected by ELISA and crabs of some residents were investigated. The specimens were collected on site and the freshwater snails were dissected to examine the infection of cercariae. The infections of metacercariae were examined by double-screen water washing or direct compression method. Cats and dogs’ faeces were collected and examined by washing and sieving Paragonimus eggs. Artificial animal infection experiments were conducted with Paragonimus cercariae isolated from the above 3 crabs. At the same time observe the natural ecological environment of the foci. Results Paragonimus westermani, Paragonimus westermani and Paragonimus spp. Were found in 3 counties, Youxi County, Yongtai County, Fujian Province. The positive rates of seroprevalence among residents in Youlin Village, Youxi County and Chishui Village, Yongtai County were 6.8% (21/309) and 6.8% (9/133) respectively. See 4 species of freshwater snails, of which 2 kinds of snails and 1 species of snails are the new first intermediate hosts of Paragonimus spp. See freshwater crab 4 species, of which 1 species of the South China Sea Paragonimus new second host. The infection rates of paragonimus spp. In the snails and crabs in Youxi County were 2.1% (27/1 344) and 92.1% (58/63), respectively. The infection index of crabs in the crabs was 171.9. Opiate positive detection rate of 1/7. In Yongtai county, the infection rates of Paratyphoid and Paragonimus spp. Were 0.6% (4/690) and 0.1% (2/2 330), respectively. The infection rate of crabs was 18.2% (18/99) Infection index 9.12. The infection rates of Paragonimus in Pinggou County were 0.3% (3/1 092) and 44.9% (35/78) respectively, and the infection index of crabs was 6.16. Conclusion Paragonimus westermani, Paragonimus and Paragonimus spp. Were found in 3 sites of Fujian Province, such as Youxi, Yongtai and Pinghe, which were highly endemic or highly infected Paragonimus foci.
