目的:探讨α-硫辛酸对早期糖尿病肾病(DN)患者的保护作用及其机制。方法:选取40例早期DN患者为治疗组,在常规治疗基础上加用α-硫辛酸(600mg/d),取同期查体健康者40例为对照组,治疗前及治疗6个月后检测患者空腹血糖(FBG)、糖化血红蛋白(HbA1c)、血肌酐(Scr)、尿微量蛋白(mAlb)、血清超氧化物歧化酶(SOD)、丙二醛(MDA)、肿瘤坏死因子(TNF-α)、白细胞介素-6(IL-6)、超敏-C反应蛋白(hs-CRP)等指标,进行对比分析。结果 :与对照组比较,治疗前DN患者的FBG、HbA1c、mAlb、MDA、TNF-α、IL-6、hs-CRP升高,SOD水平下降,差异具有统计学意义(P<0.01);采用α-硫辛酸治疗6个月后,DN患者的FGB、HbA1c、Scr较治疗前无明显变化,但mAlb、MDA、TNF-α、IL-6、hsCRP水平下降,SOD水平升高,差异具有统计学意义(P<0.01)。结论:α-硫辛酸对早期DN患者具有保护作用,推测其机制与抑制氧化应激及降低炎症水平有关。
Objective: To investigate the protective effect of α-lipoic acid on patients with early diabetic nephropathy (DN) and its mechanism. Methods: Forty patients with early-stage DN were selected as the treatment group. Α-lipoic acid (600 mg / d) was given on the basis of conventional treatment. Forty patients with normal physical examination during the same period were selected as the control group. Before treatment and 6 months after treatment, The levels of fasting blood glucose (FBG), HbA1c, Scr, mAlb, serum superoxide dismutase (SOD), malondialdehyde (MDA), tumor necrosis factor ), Interleukin-6 (IL-6), hs-CRP and other indicators were compared and analyzed. Results: Compared with the control group, the levels of FBG, HbA1c, mAlb, MDA, TNF-α, IL-6 and hs-CRP were increased and the level of SOD was decreased in DN patients before treatment (P <0.01) After 6 months treatment with α-lipoic acid, the levels of FGB, HbA1c and Scr in DN patients did not change significantly after treatment, but the levels of mAlb, MDA, TNF-α, IL-6 and hsCRP decreased and SOD levels increased Significance (P <0.01). CONCLUSION: Alpha-lipoic acid has a protective effect on early DN patients, suggesting that its mechanism is related to inhibition of oxidative stress and reduction of inflammation.
