目的探讨胎儿上纵隔左右锁骨下动脉切面及其应用价值。方法将胎儿脊柱显示至3点钟或9点钟,在胎儿三血管气管切面的基础上,向胎儿头侧稍稍偏转探头,再叠加彩色多普勒血流成像,将彩色标尺调低,并将彩色增益适当调高,即可以显示上纵隔左右锁骨下动脉切面。结果 100例正常胎儿上纵隔左右锁骨下动脉切面的显示率为100%,显示内容包括:(1)左、右锁骨下动脉走行较弯曲,呈“弯弓状”分别走向左、右肩;(2)右锁骨下动脉在气管的前方走向右肩;(3)右锁骨下动脉与右无名静脉之间的间隙较小;(4)左、右无名静脉走行平直,汇入右上腔静脉。2例迷走右锁骨下动脉表现:右锁骨下动脉走行平直,在气管的后方走向右肩;右锁骨下动脉与右无名静脉之间的间隙加大。结论胎儿上纵隔左右锁骨下动脉切面对显示左、右锁骨下动脉及其与气管的位置关系具有重要的作用,在产前常规探查有助于提高迷走右锁骨下动脉的检出率。
Objective To investigate the fetal mediastinum lateral subclavian artery section and its application value. Methods The fetal spine was displayed at 3 o’clock or 9 o’clock. On the basis of the fetal trigeminal tracheal section, the probe was slightly deflected to the fetal head and then color Doppler flow imaging was added. The color scale was lowered and the Appropriate increase in color gain, which can show the mediastinum left and right subclavian artery section. Results 100 cases of normal fetus on the mediastinum left and right subclavian artery section display rate of 100%, the display includes: (1) Left and right subclavian artery walking more curved, was “curved bow” to the left and right shoulder ; (2) the right subclavian artery in front of the trachea to the right shoulder; (3) the gap between the right subclavian artery and the right anonymous vein is small; (4) left and right anonymous vein running straight into the upper right chamber vein. 2 cases of vagus right subclavian artery performance: Right subclavian artery walking straight, in the trachea to the right shoulder; the gap between the right subclavian artery and the right anonymous vein increased. Conclusion Fetal superior mediastinum left and right subclavian arterial slices can play an important role in displaying the left and right subclavian arteries and their relationship with the trachea. Prenatal routine exploration can improve the detection rate of the right vagus subclavian artery.
