目的:比较抑郁症患者血清补体C1q、甲状腺激素(血清FTn 3、FTn 4、TSH)与正常人群的差异,及其与抑郁症发病的相关性。n 方法:选取275例抑郁症患者(抑郁症组)和275名健康体检人员(健康对照组),比较两组血清C1q、FTn 3、FTn 4、TSH水平。比较不同年龄、性别、病程、汉密尔顿抑郁量表(HAMD)评分患者的血清C1q、FTn 3、FTn 4、TSH水平差异。进一步使用回归分析评估血清C1q、FTn 3、FTn 4、TSH水平对抑郁症患者发病及疾病严重程度的影响。n 结果:抑郁症组[C1q:203(165,239)mg/L,FTn 3:(4.39±0.70)pmol/L,TSH:1.69(1.17,2.46)mIU/L],血清C1q、FTn 3、TSH水平显著低于健康对照组[C1q:(236.25±27.06)mg/L,FTn 3:4.61(4.29,4.95)pmol/L,TSH:2.04(1.42,3.01)mIU/L](均n P<0.01)。抑郁症组血清指标比较,男性[C1q :188.00 (164.00,221.00) mg/L,TSH:1.52(1.13,2.16)mIU/L]C1q和TSH水平显著低于女性[C1q :213.00 (168.25,247.75) mg/L,TSH:1.85(1.28,2.57)mIU/L](n P<0.05),男性[FTn 3:4.64 (4.23,5.06) pmol/ L,FTn 4:16.76(15.05,18.20)pmol/L]FTn 3和FTn 4水平显著高于女性[FTn 3:4.34 (3.82,4.72) pmol/L,FTn 4:15.92(14.35,17.40)pmol/L](n P<0.05)。血清C1q[n B=-0.020,n P<0.01,n OR(95%n CI):0.980(0.975,0.985)]、FTn 3[n B=-0.576,n P<0.01,n OR(95%n CI):0.562(0.408,0.775)]、TSH水平[n B=-0.274,n P<0.01,n OR(95%n CI):0.761(0.648,0.893)]是抑郁症发病的影响因素。n 结论:血清C1q及甲状腺激素可能参与了抑郁症的发病,并可能受性别因素的影响。“,”Objective:To explore the correlation between serum C1q, thyroid hormone (serum FTn 3, FTn 4, TSH) and depression by detecting the difference of serum C1q, thyroid hormone (serum FTn 3, FTn 4, TSH) between depression patients and normal people.n Methods:A total of 275 depressive patients(depression group) and 275 healthy controls(healthy group) were recruited.The serum levels of C1q, FTn 3, FTn 4 and TSH were compared between the two groups.The serum levels of C1q, FTn 3, FTn 4 and TSH in depression patients with different age, gender, course of disease and HAMD were compared.Further regression analysis was used to evaluate the effects of serum C1q, FTn 3, FTn 4 and TSH levels on the incidence and severity of depression in patients with depression.n Results:Serum C1q, FTn 3, TSH of depression group(C1q: 203(165, 239)mg/L, FTn 3: (4.39±0.70)pmol/L, TSH: 1.69(1.17, 2.46)mIU/L) were significantly lower than those of healthy group(C1q: (236.25±27.06)mg/L, FTn 3: 4.61(4.29, 4.95)pmol/L, TSH: 2.04(1.42, 3.01)mIU/L)(all n P<0.01). Compared with the serum indexes in depression group, the level of C1q and TSH in men(C1q : 188.00 (164.00, 221.00) mg/L, TSH: 1.52(1.13, 2.16)mIU/L) were significantly lower than those in women(C1q : 213.00 (168.25, 247.75) mg/L, TSH: 1.85(1.28, 2.57)mIU/L)(n P<0.05), and the level of FTn 3 and FTn 4 in men(FTn 3: 4.64 (4.23, 5.06) pmol/ L, FTn 4: 16.76(15.05, 18.20)pmol/L) were significantly higher than those in women (FTn 3: 4.34 (3.82, 4.72) pmol/L, FTn 4: 15.92(14.35, 17.40)pmol/L). Serum C1q (n B=-0.020, n P<0.01, OR95%n CI: 0.980 (0.975, 0.985)), FTn 3 (n B=-0.576, n P<0.01, OR95%n CI: 0.562 (0.408, 0.775)), TSH (n B=-0.274, n P<0.01, OR95%n CI: 0.761 (0.648, 0.893)) level were the influencing factors of depression.n Conclusion:Serum C1q and thyroid hormone may be involved in the pathogenesis of depression, and may be affected by gender factors.
