目的:探讨正常志愿者行Gd-EOB-DTPA动态增强MRI中,Gd-EOB-DTPA在十二指肠内及胆囊内显影的时间,为胆道系统功能障碍疾病的诊断提供依据。方法:前瞻性纳入30例行Gd-EOB-DTPA MRI增强扫描的志愿者,于注射对比剂后第5~90min内连续动态扫描。扫描时间点分别为第5~30min内每间隔1min、第30~60min内每间隔5min、第60~90min内每间隔10min采集一次图像。由2位放射科医生对图像进行共同评估,记录对比剂分别进入十二指肠及胆囊的时间。结果:Gd-EOB-DTPA进入十二指肠的平均时间为(30±23)min,其中注射Gd-EOB-DTPA延迟扫描20min时,对比剂进入十二指肠者所占比例为50.0%(15/30);延迟30min时所占比例为70.0%(21/30);延迟60min时所占比例为90.0%(27/30)。对比剂进入胆囊的中位时间为10min(6~29min),其中注射Gd-EOB-DTPA延迟扫描15min时,对比剂进入胆囊者所占比例为83.3%(25/30);延迟20min时所占比例为93.3%(28/30);延迟30min时所占比例为100%(30/30)。对比剂进入十二指肠及胆囊时间与年龄无关(P>0.05)。结论:正常人行Gd-EOB-DTPA MRI增强扫描中,对比剂通过十二指肠乳头进入十二指肠的时间<60min,而通过胆囊管进入胆囊的时间<30min,此时间不仅可为Gd-EOB-DTPA MRI增强检查扫描时间方案的制定提供指导,同时可为临床上怀疑Oddi括约肌功能障碍或急性胆囊炎的排除诊断提供依据。
Objective: To investigate the time of Gd-EOB-DTPA development in duodenum and gallbladder in Gd-EOB-DTPA dynamic contrast-enhanced MRI in normal volunteers and to provide evidence for the diagnosis of biliary dysfunction. Methods: Thirty consecutive volunteers who underwent enhanced Gd-EOB-DTPA MRI scans were prospectively enrolled. Dynamic contrast scanning was performed continuously within 5-90 min after contrast injection. Scan time points were the first 5 ~ 30min every interval of 1min, within 30 ~ 60min every interval of 5min, 60 ~ 90min every 10min image acquisition. Two radiologists performed a joint evaluation of the images and recorded the time when the contrast agent entered the duodenum and the gallbladder, respectively. Results: The average time for entering Gd-EOB-DTPA into the duodenum was (30 ± 23) min, with the contrast agent entering the duodenum at 20 min after injection of Gd-EOB-DTPA at a delay of 50.0% 15/30), 70.0% (21/30) with a delay of 30 minutes and 90.0% (27/30) with a delay of 60 minutes. The median time for the contrast agent to enter the gallbladder was 10 min (6 to 29 min), with 83.3% (25/30) of the contrast agent entering the gallbladder at a delay of 15 min after injection of Gd-EOB-DTPA. When the delay was 20 min The proportion was 93.3% (28/30); the delay was 30min when the proportion was 100% (30/30). Contrast agent into the duodenum and gallbladder age has nothing to do with age (P> 0.05). CONCLUSION: In contrast, Gd-EOB-DTPA MRI contrast-enhanced MRI in normal subjects shows that the time for the contrast agent to enter the duodenum through the duodenal papilla is less than 60 minutes and the time for the gallbladder to enter the gallbladder through the cystic duct is less than 30 minutes, EOB-DTPA MRI enhanced scan time program to provide guidance, but also for the clinical suspicion of Oddi sphincter dysfunction or acute cholecystitis to provide the basis for the diagnosis.
