为了评价多平面经食管超声心动图(MTEE)定性和定位诊断房间隔缺损(ASD)的价值,我们在27例均由手术证实的ASD患者中进行了经胸超声心动图(TTE)和MTEE的系统研究。其中原发孔型ASD3例,继发孔型ASD21例(其中4例患者具有2个ASD),静脉窦型ASD3例(均合并部分肺静脉畸形引流)。结果显示,3例原发孔型ASD,TTE和MTEE与手术的诊断符合率为100%;21例继发孔型ASD的25个ASD,MTEE与手术的诊断符合率为100%,TTE与手术的诊断符合率为88%,缺损直径≤5mm时,TTE与手术的诊断符合率为0%(0/1),缺损直径5~10mm时,TTE与手术的诊断符合率为75%(3/4);缺损直径>10mm时,TTE与手术的诊断符合率为95%(19/20)。3例静脉窦型ASD,MTEE与手术的诊断符合率为100%,TTE与手术的诊断符合率为33.3%。3条部分肺静脉畸形引流,MTEE与手术的诊断符合率为66.7%,TTE未发现部分肺静脉畸形引流,TTE与手术的诊断符合率为0%。显然对于小型继发孔型ASD、多发性ASD、静脉窦型ASD及合并的部分肺静脉畸形引流,MTEE的诊断价值明显高于TTE?
To assess the value of multiplane transesophageal echocardiography (MTEE) in the diagnosis and localization of atrial septal defect (ASD), we performed transthoracic echocardiography (TTE) and MTEE in 27 patients with ASD confirmed by surgery system Research. Among them, there were 3 cases of primary orifice type ASD, 21 cases of secondary orifice type ASD (4 of them had 2 ASDs) and 3 cases of sinusoid ASD (all of which were associated with partial pulmonary venous anomalies drainage). The results showed that the coincidence rate of three cases of primary perforation ASD, TTE and MTEE with the diagnosis of surgery was 100%. The 25 ASDs of 21 cases with secondary perforation ASD had 100% coincidence with the diagnosis of MTEE, The diagnostic accuracy of TTE was 75% (3/10) when the diameter of defect was less than or equal to 5mm, the coincidence rate of TTE and operation was 0% (0/1) and the diameter of defect was 5 ~ 10mm 4). When the diameter of defect was> 10mm, the coincidence rate of TTE and operation was 95% (19/20). The coincidence rate of sinus venous ASD and MTEE was 100% and the coincidence rate of TTE and operation was 33.3%. Three partial pulmonary venous anomalies were drained. The coincidence rate of MTEE and operation was 66.7%. No pulmonary venous anomalies were found in TTE. The coincidence rate of TTE and operation was 0%. Obviously, the diagnostic value of MTEE is significantly higher than that of TTE in small subdural ASD, multiple ASD, sinusoid ASD and combined partial pulmonary venous anomalies.
