目的:探讨经颅彩色多普勒超声(transcranial color-coded duplex sonography,TCCS)对脑动脉狭窄与闭塞的诊断价值。方法:对临床上怀疑有脑血管疾病的70例患者行TCCS和数字减影血管造影(DSA)检查,以DSA作为金标准,两者进行比较分析。结果:TCCS诊断脑动脉狭窄与闭塞的真实性指标灵敏度、特异度、假阳性率、假阴性率、准确度、阳性预测值、阴性预测值、阳性似然比、阴性似然比、约登指数分别为75.0%,81.6%,18.4%,25.0%,78.6%,77.4%,79.5%,4.1,0.3,0.57;可靠性指标符合率为78.6%、卡帕值为0.57。结论:TCCS具有一定的诊断真实性与可靠性,可作为脑动脉狭窄与闭塞的早期筛检性诊断方法。
Objective: To investigate the value of transcranial color-coded duplex sonography (TCCS) in the diagnosis of cerebral stenosis and occlusion. Methods: TCCS and digital subtraction angiography (DSA) were performed in 70 patients with clinically suspected cerebrovascular disease. DSA was used as the gold standard for comparative analysis. Results: The accuracy, specificity, false positive rate, false negative rate, accuracy, positive predictive value, negative predictive value, positive likelihood ratio, negative likelihood ratio, Youden index of TCCS diagnosis of cerebral artery stenosis and occlusion Respectively, 75.0%, 81.6%, 18.4%, 25.0%, 78.6%, 77.4%, 79.5%, 4.1,0.3,0.57 respectively. The coincidence rate of reliability index was 78.6%, and the kappa number was 0.57. Conclusion: TCCS has certain diagnostic authenticity and reliability and can be used as an early screening diagnostic method for cerebral stenosis and occlusion.
