目的:探讨前列腺腺癌患者年龄与Gleason评分(Gleason score,GS)的相关性。方法:研究674例患者年龄和GS均值以及不同组别均值;不同GS和年龄组别所占比例;年龄与GS、主要分级以及次要分级的相关性。结果:患者年龄25~96(70.22±8.26)岁,Gleason 6分、7分、8分、9分及10分组年龄分别为(69.06±8.35)、(70.55±8.16)、(70.99±6.54)、(71.56±9.18)岁及(72.79±11.36)岁。GS(7.08±1.09)分,主要分级(3.54±0.72)级,次要分级(3.53±0.66)级;<60、60~69、70~79及≥80岁组GS分别为(6.86±1.10)、(6.99±1.10)、(7.08±1.04)分及(7.38±1.23)分。Gleason 6分、7分以及≥8分分别占37.7%、34.3%以及28.0%。患者年龄<60岁组占10.5%(71/674),60~69岁组占30.6%(206/674),70~79岁组占47.6%(321/674),≥80岁组占11.3%(76/674)。患者年龄与GS具有显著相关性(r2=0.013,P=0.003),与主要分级具有显著相关性(r2=0.014,P=0.002),与次要分级无显著有相关性(r2=0.005,P=0.055)。结论:在前列腺腺癌患者中≥70岁者所占比例较高;GS≥7分所占比例较高;但年龄与GS具有相关性,年龄对GS的预测价值不大。
Objective: To investigate the relationship between age and Gleason score (Gleason score, GS) in patients with prostate cancer. METHODS: The age and GS mean of 674 patients and mean of different groups were studied; the proportions of different GS and age groups; and the associations of age with GS, major grade, and minor grade. Results: The age of patients was 25-96 (70.22 ± 8.26) years old. The Gleason scores at the 6th, 7th, 8th, 9th and 10th groups were (69.06 ± 8.35), (70.55 ± 8.16) and (70.99 ± 6.54) (71.56 ± 9.18) years and (72.79 ± 11.36) years. GS was (7.08 ± 1.09) points, the main grade was (3.54 ± 0.72), the second grade was (3.53 ± 0.66). The GS of <60,60-69,70-79 and ≥80 years old group was (6.86 ± 1.10) , (6.99 ± 1.10), (7.08 ± 1.04) points and (7.38 ± 1.23) points respectively. Gleason 6 points, 7 points and ≥ 8 points accounted for 37.7%, 34.3% and 28.0% respectively. Patients aged <60 years accounted for 10.5% (71/674), 60-69 years old accounted for 30.6% (206/674), 70-79 years old accounted for 47.6% (321/674), ≥ 80 years old accounted for 11.3% (76/674). There was a significant correlation between age and GS (r2 = 0.013, P = 0.003), and significant correlation with major grade (r2 = 0.014, P = 0.002) = 0.055). Conclusion: The proportion of patients ≥70 years old is higher in patients with prostatic adenocarcinoma; the proportion of patients with GS≥7 is higher; but the age is related to GS, and the age is not significant to the prediction of GS.
