目的 探讨乳房早发育与真性和假性早熟的关系。方法 采用回顾性研究的方法 ,对 36名乳房早发育患者的临床表现、盆腔B超、血激素、骨龄、身高等指标进行对照分析。结果 真性性早熟 17例 ,假性性早熟 19例 ,两组临床表现无明显差别 ;真性性早熟组骨龄 /身高年龄高于假性性早熟组 ;94.74%的真性性早熟腹部B超检查卵巢有发育 ;两组大部分患者黄体生成素 (LH)和卵泡刺激素 (FSH)正常 ,而促性腺激素释放激素 (LHRH)刺激后真性性早熟组LH呈高反应 ,FSH部分反应 ,假性组LH无反应 ,FSH呈高反应 ,FSH部分反应 ,假性组LH无反应 ,FSH低反应 ,真性组E2比假性组高 ,但两组均有 5 8.5 %和 94.7%正常。结论 乳房早发育的鉴别需要借助骨龄、腹部B超和LHRH刺激试验 ,LHRH刺激后LH和骨龄 /身高年龄对真性性早熟的诊断极为重要
Objective To investigate the relationship between early breast development and true and pseudo-precocious puberty. Methods A retrospective study was conducted to analyze the clinical manifestations, pelvic ultrasound, blood hormone, bone age and height of 36 breast implants. Results There were 17 cases of true precocious puberty and 19 cases of pseudo - precocious puberty. There was no significant difference in the clinical manifestations between the two groups. The age of bones / height of true precocious puberty group was higher than that of pseudo - precocious puberty group. 94.74% (LH) and follicle stimulating hormone (FSH) were normal in most of the two groups, and LH in true precocious puberty group was highly responsive after FSH stimulation No response, FSH showed high response, FSH partial response, pseudo-group LH no response, FSH low response, true group E2 higher than the pseudo-group, but both 8.55% and 94.7% normal. Conclusion The differential diagnosis of early breast development needs bone age, abdominal B ultrasound and LHRH stimulation test. After LHRH stimulation, LH and bone age / height age are very important for the diagnosis of true precocious puberty
