腹腔镜与开腹直肠癌全系膜切除术淋巴结清扫效果的比较

目的比较腹腔镜与开腹直肠癌全系膜切除术的淋巴清扫效果。方法回顾性分析2010年10月至2013年7月间125例直肠癌根治术患者的临床资料,其中腹腔镜组63例,开腹组62例,比较两组患者淋巴结清扫情况。结果两组患者检出淋巴结总数及各站淋巴结数比较,差异无统计学意义(P>0.05)。腹腔镜组患者手术时间明显长于开腹组,但是切口长度、术中出血量、肠道排气时间及术后住院时间均明显少于开腹组,差异有统计学意义(P<0.05)。结论腹腔镜直肠癌全系膜切除术可以获得开腹手术相似的淋巴结清扫效果,但是腹腔镜手术具有微创的优势。 Objective To compare the lymphadenectomy results of laparoscopic and open radical mastectomy. Methods The clinical data of 125 patients undergoing radical resection of rectal cancer between October 2010 and July 2013 were retrospectively analyzed. Among them, 63 patients in laparoscopic group and 62 patients in open group were compared with lymph node dissection in both groups. Results There was no significant difference in the total number of lymph nodes and the number of lymph nodes in each group between the two groups (P> 0.05). The operation time of laparoscopic group was significantly longer than that of open group, but the incision length, intraoperative blood loss, intestinal exhaust time and postoperative hospital stay were significantly less than those of laparotomy group. The difference was statistically significant (P <0.05). Conclusions Laparoscopic total mesorectal excision can achieve the similar effect of laparotomy in lymph node dissection, but laparoscopic surgery has the advantages of minimally invasive surgery.
