目的利用质子磁共振波谱(1H-MRS)技术研究帕金森病(PD)患者壳核的代谢变化及意义。方法选择15例PD患者作为PD组,依据Hoehn和Yahr分期,将PD组分成早期PD组(H/Y分级<Ⅱ级)和晚期PD组(H/Y分级≥Ⅱ级),另选择13例健康志愿者作为对照组。3组分别予以双侧壳核1H-MRS检查,测定双侧壳核N-乙酰天门冬氨酸/肌酸(NAA/Cr)、N-乙酰天门冬氨酸/胆碱复合物(NAA/Cho)和胆碱复合物/肌酸(Cho/Cr)值。结果早期PD组患者NAA/Cr、NAA/Cho、Cho/Cr值分别为(1.3±0.3)、(1.16±0.30)和(1.13±0.27),晚期PD组分别为(1.2±0.5)、(1.14±0.44)、(1.16±0.25),对照组分别为(2.3±0.4)、(1.64±0.28)、(1.43±0.32)。早期PD组和晚期PD组NAA/Cr、NAA/Cho值与对照组比较差异有统计学意义(P<0.05)。结论1H-MRS技术可以无创性地为PD患者双侧壳核内神经代谢改变提供有价值的信息。
Objective To study the metabolic changes of putamen in patients with Parkinson’s disease (PD) by proton magnetic resonance spectroscopy (1H-MRS). Methods Fifteen patients with PD were selected as PD group. According to Hoehn and Yahr stages, PD patients were divided into early PD group (H / Y grade <Ⅱ grade) and advanced PD group (H / Y grade> II grade) Healthy volunteers as a control group. The three groups were examined by 1H-MRS of bilateral putamen respectively. The levels of N-acetyl aspartate / creatine (NAA / Cr), NAA / Cho ) And choline complex / creatinine (Cho / Cr) values. Results The NAA / Cr, NAA / Cho and Cho / Cr values in the early PD group were (1.3 ± 0.3) and (1.16 ± 0.30) and ± 0.44), (1.16 ± 0.25) in the control group and (2.3 ± 0.4), (1.64 ± 0.28) and (1.43 ± 0.32) in the control group respectively. The NAA / Cr and NAA / Cho values in the early PD group and the late PD group were significantly different from those in the control group (P <0.05). Conclusion 1H-MRS can provide valuable information for noninvasive changes of neurological metabolism in bilateral putamen in patients with PD.
