目的研究分析血人绒毛膜促性腺激素(HCG)比率预测不明部位妊娠结局的临床价值。方法选择不明部位妊娠患者180例作为研究对象,在就诊当天和48 h后对患者的β-人绒毛膜促性腺激素(β-HCG)进行检测,对HCG比率(HCG48/HCG0)进行测算,且对患者应用B超进行连续监测,评价血HCG比率预测该病患者妊娠结局价值。结果 180例患者中,142例患者为正常宫内妊娠,22例患者为异位妊娠,16例为自然流产,三种患者停经时间和年龄两两比较差异无统计学意义(P>0.05)。142例正常宫内妊娠患者的血HCG比率范围1.58~3.99,平均(2.24±0.35),其中122例血HCG比率≥2;22例异位妊娠患者的血HCG比率范围0.75~2.68,平均(1.50±0.22),其中13例血HCG比率在0.87~2;16例自然流产患者的血HCG比率范围0.59~1.71,平均(0.77±0.12),其中11例血HCG比率<0.87。正常宫内妊娠患者血HCG比率最高,异位妊娠患者血HCG比率比自然流产患者高,三种患者的血HCG比率两两比较差异具有统计学意义(P<0.05)。结论血HCG比率预测不明部位妊娠结局的临床价值较高,应推广采纳。
Objective To study the clinical value of blood human chorionic gonadotropin (HCG) ratio in predicting the outcome of unknown pregnancy. Methods One hundred and eighty pregnant women with unknown sites were enrolled in this study. Β-HCG (HCG48 / HCG0) was measured on the day of treatment and after 48 h, and the HCG48 / HCG0 ratio was calculated. B-monitoring of patients with continuous monitoring of blood HCG ratio to predict the outcome of the patient’s pregnancy outcome. Results Of the 180 patients, 142 were intrauterine pregnancies, 22 were ectopic and 16 were spontaneous abortion. There was no significant difference in the menopause time and age between the two groups (P> 0.05). The blood HCG ratio in 142 cases of normal intrauterine pregnancy ranged from 1.58 to 3.99, with an average of 2.24 ± 0.35, of which 122 cases had HCG≥2. The blood HCG rates in 22 cases of ectopic pregnancy ranged from 0.75 to 2.68, ± 0.22). The ratio of HCG in 13 cases was 0.87 ~ 2. The HCG ratio in 16 cases of spontaneous abortion ranged from 0.59 to 1.71, with an average of 0.77 ± 0.12, of which 11 cases had HCG ratio of <0.87. The blood HCG ratio of patients with normal intrauterine pregnancy is the highest, while the blood HCG rate of patients with ectopic pregnancy is higher than that of spontaneous abortion. The blood HCG ratios of the three patients have statistically significant differences (P <0.05). Conclusion The clinical value of predicting the pregnancy outcome with unknown HCG rate is high and should be widely adopted.
