目的探讨膀胱癌雌激素受体启动子(ERα)CpG岛甲基化与ER基因表达的相关性。方法 35例膀胱癌标本,以免疫组化SP法分析其ER基因的表达,23例以甲基化特异性PCR(MS-PCR)进行了ERα基因CpG岛甲基化分析。结果 35例膀胱癌中ER总阳性率为28.6%;随病理分级的增加,ER表达降低。23例膀胱癌ERα启动子CpG岛完全甲基化发生率43.48%(10/23),发生部分甲基化为13.04%(3/23),未甲基化为43.48%(10/23)。以完全甲基化进行分析,膀胱癌Ⅰ级ERα基因甲基化发生率20.00%(1/5),Ⅱ级46.47%(7/15),Ⅲ级66.67%(2/3)(P>0.05);正常膀胱组织ERα基因CpG岛未甲基化。ER阳性表达与ERα甲基化呈负相关。结论膀胱癌中存在有ERα基因的甲基化,并且与ER的表达呈负相关。ERα基因CpG岛甲基化可能是膀胱癌中ER表达失活的分子机制。
Objective To investigate the relationship between methylation of ERα CpG island and ER gene expression in bladder cancer. Methods Totally 35 cases of bladder cancer were examined by immunohistochemical SP method. The methylation status of ERα gene CpG island in 23 cases was analyzed by methylation-specific PCR (MS-PCR). Results The positive rate of ER in 35 cases of bladder cancer was 28.6%. The ER expression was decreased with the increase of pathological grade. The complete methylation rate of CpG island of ERα promoter in 23 cases of bladder cancer was 43.48% (10/23), partial methylation was 13.04% (3/23) and unmethylation was 43.48% (10/23). The methylation rate of grade Ⅰ ERα gene in bladder cancer was 20.00% (1/5), grade Ⅱ 46.47% (7/15), grade Ⅲ 66.67% (2/3) (P> 0.05) ); Normal bladder tissue CpG island ERα gene unmethylation. ER positive expression and ERα methylation was negatively correlated. Conclusion There is a methylation of ERα gene in bladder cancer and a negative correlation with the expression of ER. CpG island methylation of ERα gene may be the molecular mechanism of inactivation of ER expression in bladder cancer.
